Objective: to analyze the effect of the nursing model based on the transtheoretical model of behavior(TTM)on the negative emotion and quality of life of elderly patients with atrial fibrillation,and to clarify its application effect,so as to provide some reference for the relevant departments of the hospital to formulate corresponding nursing measures in the future,so as to improve the clinical nursing level and promote the rehabilitation of patients.Methods: the research type of this study was an unequal control design similar to experimental research.According to the inclusion criteria and exclusion criteria,164 elderly patients with atrial fibrillation hospitalized in a third class a hospital in Weihai City,Shandong Province from February to July 2021 were included.The control group and the experimental group were set up.76 cases in the control group were given routine nursing;88 cases in the experimental group were treated with the nursing method based on the nursing model of behavior change theory.Before the intervention,one month,three months and six months after the intervention,the self-assessment scale of anxiety and depression and the short form of quality of life were used to carry out the questionnaire,and SPSS 26.0 software was used for statistical analysis to test the level α= 0.05,P < 0.05,the difference was statistically significant.Result:(1)The general conditions and general data of the subjects conform to the normal distribution.Among the 164 patients who completed this study,the age range is 60-82 years(64.61 ± 7.91),96 of them are male patients,accounting for 58.54%,68 of them are female patients,accounting for41.46%,all of them are Han patients,91 of them are technical secondary school or high school,accounting for the largest proportion,accounting for 55.48%.Medical insurance mode: the number of self funded patients is the least,accounting for 9.15%.There was no significant difference in general data between the two groups before intervention(P >0.05).(2)Before intervention,there was no significant difference in SAS scores between the control group(43.69 ± 7.44)and the experimental group(36.93 ± 5.63),SDS scores between the control group(48.54 ± 6.34)and the experimental group(47.93 ± 6.21),SF ~ 36 scores between the control group(82.61 ± 8.68)and the experimental group(86.98 ± 5.38)(P > 0.05).(3)One month after discharge,the SAS score of the patients in the control group(37.77 ± 4.76)and the experimental group(36.93 ± 5.63),the SDS score of the patients in the control group(42.67 ±6.72)and the experimental group(41.98 ± 6.69),and the SF ~ 36 score of the patients in the control group(86.05 ± 4.67)and the experimental group(86.98 ± 5.38)had no significant difference after one month of intervention(P > 0.05).(4)Three months after discharge,the SAS score of the patients in the control group(36.29 ± 3.33)and the experimental group(33.97 ± 4.37),the SDS score of the patients in the control group(41.13± 6.09)and the experimental group(36.74 ± 5.66),and the SF ~ 36 score of the patients in the control group(88.13 ± 4.16)and the experimental group(92.29 ± 4.67)were significantly different after three months of intervention(P < 0.05).(5)Six months after discharge,the SAS score of the patients in the control group(35.68 ± 4.70)and the experimental group(32.43 ± 4.87),the SDS score of the patients in the control group(37.78 ±6.65)and the experimental group(32.48 ± 4.61),and the SF ~ 36 score of the patients in the control group(89.95 ± 3.95)and the experimental group(95.02 ± 3.40)were significantly different after six months of intervention(P < 0.05).Conclusion:(1)The incidence of anxiety and depression in elderly patients with atrial fibrillation is high and the quality of life is reduced.Its distribution characteristics are affected by many factors;(2)The nursing model based on behavior transformation theory can reduce the anxiety and depression of elderly patients with atrial fibrillation,and the incidence of men is higher than that of women,and the higher education population is higher than that of other populations;(3)The nursing model based on behavior transformation theory can improve the quality of life of elderly patients with atrial fibrillation,and the quality of life of people with high cardiac function and higher education is high;(4)In nursing intervention based on behavior transformation theory,due to the poor acceptance ability of elderly patients,we can use wechat and telephone communication to give nursing intervention step by step to ensure the effectiveness of intervention. |
