Study On The Intervention Of Huazhuo And Xue Granule On Transient Ischemic Attack And The Mechanism Of Ischemic Tolerance

Objective: Based on the theory of "curing disease",this study observed the effects of Huazhuo and Xue Granules on the healing and TNF-α levels of patients with TRANSIENT ischemic attack,and explored the possible regulatory mechanism of Huazhuo and Xue Granules on the healing and cerebral ischemia tolerance of patients with transient ischemic attack.Methods: 1.Clinical study: 70 eligible patients(from the neurology outpatient department and ward of the Second Affiliated Hospital of Shandong University of Traditional Chinese Medicine)were selected and divided into control group and treatment group with 35 patients in each group by randomized control method.The control group was treated with conventional Western medicine,and the treatment group was treated with huazhuo and Blood granules on the basis of conventional Western medicine.Both groups were treated for 14 consecutive days,and the levels of TNF-α were detected by chemiluminescence before and after 14 days of treatment,followed up to 90 days later.2.Experimental study: PC12 cells cultured in vitro were randomly divided into 5 groups: 1 group(blank control group serum),2(blank serum of model group),3 groups(turbidity and blood particles containing medicine serum of model group),4 group(blank serum group)and 5(turbidity and blood particles containing medicine serum group),including control group don’t do any processing,model group and culture by oxygen lack of sugar after treatment(OGD)2 h,The experimental group underwent advanced OGD10 min to simulate ischemic preconditioning(IPC)and OGD2 h again after 24 h culture.Cell viability was detected by CCK-8 assay,m RNA expression of sir T-1 was detected by RT-PCR,and tumor necrosis factor(TNF-α)was detected by Elisa kit.Results: 1.Clinical study: the total effective rate of the treatment group was significantly better than that of the control group(P < 0.05);The TCM syndromes in treatment group were significantly improved compared with control group(P < 0.05).The serum TNF-α level in treatment group was significantly lower than that in control group(P < 0.05).2. Experimental study: cell viability: group 1 > group 5 > group 3 > group 2(P < 0.05);Group 1 > group 5 > group 4 > group 2(P < 0.05);The expression level of SIRT-1 in group 1 > group 5 > group 3 > group 2(P <0.05);Group 1 > group 5 > group 4 > group 2(P < 0.05);TNF-α content:group 2 > group 4 > group 5 > group 1(P < 0.05);Group 2 > Group 3 > Group 5 > Group 1(P < 0.05).Conclusion: 1.Clinical study: Turbidity and blood granules can improve the clinical symptoms of PATIENTS with TIA and reduce the inflammatory response of patients with TIA.2.Experimental study: The addition of huazuzhuo and blood granule containing drug serum after IPC can reduce the damage caused by OGD2 h,enhance cell viability,up-regulate sir T-1 expression and reduce TNF-α content.Comprehensive clinical and experimental studies speculated that the enhanced effect of Huazhuo and Xue granules on IT after IPC was related to the up-regulation of SIRT-1 expression and the reduction of inflammatory response.
