Objective: On the basis of using conventional therapy,to observe the clinical effect of oral administration of traditional Chinese medicine combined with external application of Bingxiao powder in the treatment of early soft tissue swelling and pain in the perioperative period of LaugeHansen supination-external rotation IV degree fractures,evaluating the efficacy and safety of oral administration of traditional Chinese medicine combined with external application in the treatment of pain and swelling in the perioperative period of fractures based on the theory of internal and external treatment,providing theoretical data for clinical treatment.Methods: The cases collected in this study were all patients with supination-external rotation IV°ankle fracture who were admitted to the Department of Traumatology and Orthopedics,The First Affiliated Hospital of Shandong University of Traditional Chinese Medicine from September 2019 to September 2021,and 60 cases were selected according to the inclusion and exclusion criteria.The patients were randomly divided into three groups on the basis of conventional treatment,namely the Jiajian Taohong Siwu Decoction group(hereinafter referred to as the control group),the Bingxiao powder external application group(hereinafter referred to as the external treatment group),and the Jiajian Taohong Siwu Decoction combined with Bingxiao powder external application group(hereinafter referred to as the combination group)was given treatment from the first day after the patient was admitted to the hospital,and was discontinued on the day of surgery.The patients were observed and monitored before treatment,on the 3rd,5th and 7th days after treatment.The monitoring indicators included the improvement of ankle swelling,pain score,preoperative waiting time and perioperative complications.The clinical data of 60 patients were compared between groups and within groups to determine whether there was a statistical difference in efficacy.Results:1.There were no significant differences in gender,age,patient distribution and the circumference of the unaffected ankle joint among the three groups of patients(P>0.05),which were comparable.2.There was no significant difference in VAS score,swelling value and perioperative complication rate among the three groups before treatment(P>0.05).3.Comparison within groups: The pain scores and ankle swelling observation indexes of the three groups of patients before treatment and on the 3rd,5th,and 7th days after treatment were compared with those before treatment,and the differences were statistically significant(P<0.05).4.On the 3rd day after treatment,there was no significant difference in VAS score,swelling degree and swelling improvement degree among the three groups(P>0.05).On the 5th and 7th day after treatment,the VAS score,swelling degree and swelling improvement degree of the combination group were better than those of the control group and the external treatment group,and the external treatment group was better than the control group in terms of swelling improvement.5.Comparison of preoperative waiting time,the difference between the three groups was statistically significant(P<0.05),and The P values of the combination group was less than 0.05 in the comparison between the groups,indicating that the combination group was better than the other two groups in shortening the preoperative waiting time.Conclusion: Compared with the single use of one of the drugs,Jiajian Taohong Siwu Decoction combined with Bingxiao powder can more effectively treat the pain of the affected limb during the perioperative period of ankle fracture and shorten the hospitalization time of patients.It can reduce the side effects of drugs,reduce the hospitalization costs of patients,reduce the swelling degree of the affected limbs,and create favorable conditions for the early recovery of patients.It is beneficial for traditional Chinese medicine to play an active role in the perioperative treatment of fractures. |
