Study On The Distribution And Correlation Of TCM Syndromes Of Rheumatoid Arthritis With Anxiety And Depression State

Objective:Through this study in patients with rheumatoid arthritis(RA)associated with anxiety depression generally,vital signs,laboratory indexes and disease activity,summarizing the rheumatoid arthritis(RA)associated with the clinical characteristics of patients with anxiety,depression,and to explore the Traditional Chinese Medicine syndrome type distribution,so as to improve the understanding of Chinese and western medicine of patients with RA It provides certain reference for clinical syndrome differentiation and treatment.Methods:Patients diagnosed with rheumatoid arthritis with anxiety and depression in Shandong Hospital of Traditional Chinese Medicine from November 2020 to May 2021 were selected as subjects.Basic information and TCM syndrome information of 148 RA patients with anxiety and depression were collected through questionnaire survey,to understand the influencing factors of anxiety and depression and TCM syndrome type distribution,and to explore the correlation of syndrome type distribution with gender,age,education,disease activity,etc.After the questionnaire results were carefully reviewed by the investigator,SPSS23.0 statistical software was used for statistical analysis of the obtained data to analyze whether the relevant data had statistical significance.Results:1.Influencing factors of anxiety and depression in RA patients with anxiety and depression:Gender,age,syndrome type,CRP,Rheumatoid factor(RF),anti-cyclic peptide containing Citrulline antibody(anti-CCP),Erythrocyte sedimentation Rate(ESR),tenderness,swelling,Visual Analogue Score(VAS),disease activity,hormone use,chronic disease complications,The difference was statistically significant(P<0.05),and education level had no statistical significance in the distribution of anxiety and depression(P>0.05).Multivariate Logistic regression analysis of anxiety and depression showed that gender,age,RF,CCP,ESR,number of swollen joints,disease activity and comorbidity were correlated with the occurrence of anxiety and depression(P<0.05),while syndrome type,CRP,number of tenderness joints,VAS,hormone use had no significant correlation with the occurrence of anxiety and depression(P>0.05);2.Distribution of TCM syndrome types of RA with anxiety and depression:Bizu phlegm and blood stasis syndrome(39,26.35%)>hot and humid card bizu,22.97%(34)>two qi syndrome(26,17.57%)>liver and kidney deficiency syndrome(23,15.54%)>rheumatism bizu certificate(14,9.46%)>cold dampness bizu card(12,8.11%);3.Distribution of TCM syndrome types of RA with anxiety and depression and correlation of various factors:In the distribution of TCM syndrome types,gender(χ2=24.874,P=0.000<0.05),age(χ2=29.690,P=0.001<0.05),adverse factors(χ2=88.933,P=0.000<0.05)were correlated.Disease activity(χ2=19.201,P=0.205>0.05),number of chronic diseases(χ2=12.431,P=0.900>0.05),hormone use(χ2=1.983,P=0.852>0.05),education level(χ2=2.926,P=0.711>0.05)and all laboratory indexes(P>0.05)had no correlation.Conclusion:1.In this study,the incidence of RA with anxiety and depression was 34.42%;Gender,age,RF,CCP antibody,ESR,number of swollen joints and number of chronic diseases were correlated with anxiety and depression;2.In RA patients with anxiety and depression,phlegm and stasis syndrome was the most common,accounting for 26.35%;The second was damp-heat obstruction syndrome,accounting for 22.97%;3.The TCM syndrome type distribution of RA patients with anxiety and depression was correlated with gender,age,adverse factors,disease activity,disease activity and the number of complicated chronic diseases;4.In RA patients with anxiety and depression,the main symptoms were qi and blood deficiency syndrome,phlegm and stasis obstruction syndrome and dampness and heat obstruction syndrome in females,while the main symptoms were dampness and heat obstruction syndrome and phlegm and stasis obstruction syndrome in males.In the elderly,the main symptoms are deficiency of liver and kidney,phlegm and blood stasis,and deficiency of qi and blood.In middle age,dampness and heat syndrome and phlegm and blood stasis syndrome were the main symptoms.Young people are mainly affected by rheumatism and phlegm stasis.
