Analysis Of Syndrome Type Changes And Related Risk Factors Of Yin Deficiency And Yang Hyperactivity Syndrome In 200 Cases Of Prehypertension

Objective:To explore the syndrome types and blood pressure changes of people with Yin deficiency and yang hyperactivity syndrome of normal high-value blood pressure,compare and observe the changes of biochemical indexes and the correlation of relevant risk factors among different syndrome types,so as to provide a theoretical basis for TCM syndrome differentiation of normal high-value blood pressure and early prevention of hypertension.Methods:200 subjects with Yin deficiency and yang hyperactivity syndrome with prehypertension were collected from January 2019 to June 2020 in the Department of Cardiology of the Affiliated Hospital of Shandong University of traditional Chinese medicine.They were visited 18 months from the first observation(i.e.July 2020 to December 2021),and the obtained data were statistically analyzed.Results:1.Among the 200 subjects with Yin deficiency and yang hyperactivity syndrome of prehypertension included in the study,21.5% turned to hyperactivity of liver fire,12% to excess of phlegm and dampness,and 11%to deficiency of yin and Yang during the 18 month follow-up visit.There were significant differences in fasting blood glucose,high density lipoprotein cholesterol(HDL-C),serum creatinine and left atrial anteroposterior diameter between before and after observation(P<0.05);The left ventricular anteroposterior diameter of phlegm dampness excess syndrome was significantly different from that before observation(P<0.05);The differences of HDL-C,blood homocysteine(Hcy)and left ventricular anterior posterior diameter in Yin deficiency and yang hyperactivity syndrome were statistically significant compared with those before observation(P<0.05).2.Among the 200 subjects with Yin deficiency and yang hyperactivity syndrome with prehypertension,23.5% returned to normal and 41% developed hypertension during the 18 month follow-up.There were significant differences in total cholesterol,low density lipoprotein cholesterol(LDLC),HDL-C,Hcy and left ventricular anterior posterior diameter in normal blood pressure group compared with those before observation(P<0.05),and there were significant differences in LDL-C and HDL-C in hypertension group compared with those before observation(P<0.05).3.There were significant differences in triglyceride and glomerular filtration rate among different syndrome types(P<0.05).There was significant difference in body mass index(BMI)between liver fire hyperactivity syndrome and phlegm dampness hyperactivity syndrome(P<0.01),and there was significant difference in body mass index(BMI)between phlegm dampness hyperactivity syndrome and yin-yang deficiency syndrome(P<0.01).4.Compared with different blood pressure levels,there were significant differences in cerebral infarction,dyslipidemia,blood homocysteine,lipoprotein(a)and ventricular septum(P<0.05).There were significant differences in cerebral infarction,fasting blood glucose,blood homocysteine and ventricular septum between hypertension group and normal blood pressure group(P<0.025),and there were significant differences in cerebral infarction,dyslipidemia and blood homocysteine between prehypertension group and normal blood pressure group(P<0.025).5.The results of multivariate logistic regression analysis showed that low BMI tended to liver fire hyperactivity syndrome compared with phlegm dampness hyperactivity syndrome;Compared with the syndrome of hyperactivity of yin and Yang,the syndrome of hyperactivity of yin and Yang tends to be small in the left atrium;Compared with Yin-Yang deficiency syndrome,the low glomerular filtration rate tends to yin-yang deficiency syndrome;Compared with Yin and yang deficiency syndrome,low glomerular filtration rate tends to Yin and yang deficiency syndrome.Conclusion:The changes of syndrome types of 200 subjects with prehypertension Yin deficiency and yang hyperactivity syndrome were observed.Some of them changed before and after Yin deficiency and yang hyperactivity syndrome,and their blood pressure changed.There were differences in diastolic blood pressure,serum creatinine,high density lipoprotein cholesterol and blood homocysteine between prophase and anaphase;There were differences in triglyceride and glomerular filtration rate among different syndrome types;There were differences in cerebral infarction,dyslipidemia,blood homocysteine,lipoprotein(a)and ventricular septum among different blood pressure levels.Compared with phlegm dampness syndrome,low BMI tends to liver fire hyperactivity syndrome;Compared with Yin deficiency and yang hyperactivity syndrome,the anterior and posterior diameter of small left atrium tends to yin deficiency and yang hyperactivity syndrome;Compared with Yin-Yang deficiency syndrome,the low glomerular filtration rate tends to yin-yang deficiency syndrome;Compared with Yin and yang deficiency syndrome,low glomerular filtration rate tends to Yin and yang deficiency syndrome.
