ObjectiveThe study aims to explore the distribution of syndrome elements in polycystic ovary syndrome(PCOS)through epidemiological investigation,and to analyze the correlation between the main syndrome elements and human body composition(including standard proportion of skeletal muscle,body fat rate,body mass index,waist-hip ratio,extracellular water ratio,obesity,visceral fat area,lipid accumulation index,visceral fat index)in patients with polycystic ovary syndrome.The study strives to provide objective indices for Syndrome differentiation of traditional Chinese medicine(TCM)and provide reference for it to further understand,diagnose and treat polycystic ovary syndrome.MethodThe research subjects are patients who met the inclusion criteria of the gynecology clinic in Xiamen Hospital of Traditional Chinese Medicine from January 2021 to December 2021.The general conditions and the TCM syndrome elements score table of patients were collected and inputted into the TCM Health Identification System.The top three single disease location syndrome elements and the first six single disease nature syndrome elements with the highest frequency were then obtained.The statistics were carried out by using Excel software and SPSS 23.0.The TCM syndrome elements were set as the dependent variables and the index of human body composition was selected as the independent variable.A binary Logistic regression was then established to analyze the correlation between TCM syndrome elements and human body composition.Results1.There were 230 effective data in this experiment,among which the youngest was 18 years old,the oldest was 41 years old and the median age was 26.5 years old.The data were grouped by 7-years-old physiological cycle and the 21-34-year-old age group of 204 people was the most common group for the first treatment of the disease.In terms of educational level,the subjects involved in the study were mostly undergraduate students,accounting for53.48%.The occupations were mainly led by“Others”,accounting for 20.87%,followed by“Students”,“Service Industry”and“Administration”,accounting for 17.39%,11.74%and11.74%,respectively.2.TCM disease location syndrome elements of PCOS were mainly concentrated in the liver(186 cases,80.87%),followed by the spleen(115 cases,50.00%)and the kidney(85 cases,36.96%).Disease nature syndrome elements were distributed in the order of 192 cases of Qi stagnation(83.48%),138 cases of Qi deficiency(60.00%),130 cases of Yang deficiency(56.52%),128 cases of Yin deficiency(55.65%),121 cases of blood deficiency(52.61%),and111 cases of dampness(48.26%).3.In terms of human body composition index,the median BMI was 22.00kg/cm~2;the median waist-hip ratio was 0.85;the median obesity was 105%;the median standard proportion of skeletal muscle was 92.388%;the average body fat rate was 32.32%;the median value of visceral fat area was 79.20cm~2;the average extracellular water ratio was 0.381;the median lipid accumulation index was 22.560;the median visceral fat index was 1.546.Cases were divided into three groups according to BMI.There were 28 cases with BMI lower than 18.5,accounting for 12.17%,which indicates that there were some low body weights in PCOS population.Their median standard proportion of skeletal muscle was lower than the normal reference value range,and the average body fat rate was higher than the normal reference value range.A total of 119 cases with BMI ranging from 18.5 to 24(including equal to 18.5)accounted for 51.74%.Their average standard proportion of skeletal muscle was lower than the normal reference value range,and the average body fat rate was higher than the normal reference value range.There were 83 cases with BMI higher than or equal to 24 accounting for36.09%.Their body fat rates were higher than the normal reference value range,and the skeletal muscle was in the normal reference value range.4.Logistic regression analysis showed that liver and Yin deficiency were negatively correlated with waist-hip ratio;kidney and Yang deficiency were positively correlated with extracellular water ratio;Qi deficiency and blood deficiency were positively correlated with standard proportion of skeletal muscle;dampness was positively correlated with visceral fat area.No correlation was found between TCM syndrome elements and other human body composition indices.Conclusions1.The age range of PCOS patients who comes to the gynecology hospital for the first time is21-34 years old.PCOS patients are mainly students,people in service industry,and administration and are highly educated.They are more aware of their health,so they have good feedback on the education of lifestyle intervention.2.The syndrome elements of PCOS patients are mainly concentrated in the liver,followed by spleen and kidney.Disease nature syndrome elements are mainly Qi stagnation,followed by Qi deficiency,Yang deficiency,Yin deficiency,blood deficiency and dampness.3.The human body composition indices of PCOS patients is characterized by high waist-hip ratio,high body fat rate and normal standard proportion of skeletal muscle.The detection results of human body composition provide objective reference for guiding lifestyle.4.There are correlations between the syndrome elements of PCOS and human body composition indices.Liver and Yin deficiency are correlated to waist-hip ratio;kidney and Yang deficiency are correlated to extracellular water ratio;Qi deficiency and blood deficiency correlats to standard proportion of skeletal muscle;there is a correlation between dampness and visceral fat area.The finding can offer some ideas for PCOS diagnosis and treatment in clinical syndrome differentiation and treatment. |
