| Objective:To observe the effect of Bazhen Decoction and Yangxin Decoction on postoperative delirium in elderly patients with intertrochanteric fracture of femur,and to provide a reference for clinical formulation of reasonable postoperative delirium prevention measures.Method:A total of 140 elderly patients with intertrochanteric fracture of femur who were admitted to six orthopaedic trauma departments of Fuzhou Second Hospital from February 2021 to January 2022 and met the inclusion and exclusion criteria were selected.The included patients were divided into treatment group and control group by random number table method,70 cases in each group.The control group was given modern medical preventive measures,including(effective analgesia,good sleep,prevention of dehydration and constipation,adjustment of blood pressure and blood glucose abnormalities,correction of electrolyte disorders,improvement of hypoxemia,avoiding the use of drugs easy to induce delirium,early postoperative functional exercise),On the basis of modern medicine preventive measures in the control group,the experimental group was added with Bazhen Decoction and Yangxin Decoction from admission to 7 days after operation.Assess The occurrence of Delirium using The Confusion Assessment Method(CAM),assess The severity of Delirium using The Delirium Rating Scale(DRS-R-98).Hemoglobin content was collected on the day after admission and the 1st,3rd and 7th day after operation.Statistical software SPSS 20.0 was used for data analysis.Result:1.Comparison of general data: There were no statistically significant differences between the control group and the experimental group in gender,age,body mass index,preoperative preparation time,operation time and blood transfusion situation(P > 0.05),and the two groups were comparable.2.Incidence of postoperative delirium: in the control group,postoperative delirium occurred in 16 cases,with an incidence of 23.52%,and in the experimental group,postoperative delirium occurred in 7 cases,with an incidence of 11.29%.The incidence of postoperative delirium in experimental group was lower than that in control group,and the difference between the two groups was statistically significant(P < 0.05).3.Comparison of postoperative delirium severity: The DRS score of postoperative delirium severity score in experimental group(24.00±3.55)was lower than that in control group(27.56±3.63),and the difference between the two groups was statistically significant(P < 0.05).4.Comparison of postoperative delirium types: 5 cases of excited type,8 cases of inhibitory type,and 3 cases of mixed type in the control group,and 2 cases of excited type,3cases of inhibitory type,and 2 cases of mixed type in the experimental group.There was no statistical significance in the types of delirium after postoperative delirium between the two groups(P > 0.05).5.Comparison of hemoglobin content: Comparison between groups:There was no significant difference in hemoglobin content between the two groups on the second day after admission,the first day after surgery and the third day after surgery(P > 0.05),and the hemoglobin content in the experimental group was higher than that in the control group on the seventh day after surgery,with statistically significant difference(P < 0.05).Comparison within the same group:The relationship of different time in the control group was the second day after admission > the first day after surgery > the third day after surgery = the seventh day after surgery.The relationship of different time in the experimental group was the second day after admission > the first day after surgery > the third day after surgery < the seventh day after surgery.conclusion:For the elderly patients with intertrochanteric fracture of femur,the application of Bazhen Decoction and Yangxin Decoction combined with modern medical prevention measures can effectively reduce the incidence of postoperative delirium,reduce the severity of postoperative delirium,and improve the hemoglobin content. |
