Objective:On the basis of functional exercise of quadriceps femoris,by comparing the curative effect difference of electroacupuncture and acupuncture in the treatment of Knee Osteoarthritis(KOA),to observe the effect of electroacupuncture on WOMAC arthritis index score,VAS score and s EMG signal of internal and external femoral muscles,in order to provide clinical data for electroacupuncture combined with functional exercise of quadriceps femoris in the treatment of Knee Osteoarthritis.Methods:According to the clinical treatment,62 cases of unilateral KOA patients were collected and randomly assigned into observation group and control group with 31 patients in each group.Among them,61 cases were actually completed due to the problems of case shedding and elimination,including 31 cases in the observation group and 30 cases in the control group.Both groups of patients received health education and quadriceps function exercise,including quadriceps function exercise once every other day,3 times a week,4 weeks of treatment,a total of 12 times.The control group was treated with conventional acupuncture.Acupuncture was applied on the affected side of Yanglingquan,Yinlingquan,Zusanli,Dubi,Neixiyan,Liangqiu and Xuehai.The needles were kept for 30 minutes each time,once every other day,three times a week,a total of 4 weeks,12 times as a course of treatmentonce;the observation group was given electroacupuncture on the basis of the control group,the needle handles of the affected side of Dubi,Liangqiu,Neixiyan and Xuehai were connected with electroacupuncture equipment.The dilatational wave(sparse wave 4Hz,dense wave 20Hz)was used,and the needles were kept for 30 minutes each time,once every other day,three times a week,a total of 4 weeks,12 times as a course of treatmentonce.The curative effect was evaluated after one course of treatment.Observe patients before and after treatment of WOMAC arthritis index score,VAS score,and the changes of s EMG signals Integrated EMG(i EMG),Median Frequency(MF)of quadriceps femoris.Results SPSS23.0 was used for statistics,and P < 0.05 was considered statistically significant.Results:1.Both groups were effective after treatment,the total effective rate was 90.32% in the observation group and 80.00% in the control group.The curative effect of the observation group was significantly better than that of the control group(P < 0.05).2.There was significant difference in WOMAC arthritis index score after treatmentin in the observation group(P < 0.01).There was significant difference in WOMAC arthritis index score after treatment in the control group(P < 0.05).There was significant difference in the decrease of WOMAC arthritis index score between the two groups(P < 0.01).Compared between the groups,the observation group was better than the control group in reducing the score of WOMAC arthritis index,and was better than the control group in pain score,joint stiffness score and daily activity score(P < 0.05).3.There was significant difference in VAS score in the observation group after treatment(P <0.01).There was significant difference in VAS score in the control group after treatment(P <0.05).There was significant difference in the decrease of VAS score between the two groups(P < 0.01).Compared between the two groups,the effect of the observation group in improving the pain symptoms of KOA patients with KOA was better than that of the control group.4.There was significant difference in the figure of s EMG signals(i EMG,MF),the i EMG difference and the MF difference between the two groups after treatment(P < 0.05).Compared with the control group,the observation group was better than the control group in increasing the figure of s EMG signals(i EMG,MF)and reducing the i EMG difference and the MF difference(P < 0.05).Conclusion:1.On the basis of functional exercise of quadriceps femoris,electroacupuncture and conventional acupuncture can improve the clinical symptoms of KOA patients,and electroacupuncture is better than conventional acupuncture.2.On the basis of functional exercise of quadriceps femoris,electroacupuncture and conventional acupuncture can enhance the muscle strength and endurance of quadriceps femoris,adjust the imbalance between muscle strength and endurance of internal and external quadriceps femoris,improve the function of knee joint and maintain the balance of knee joint.3.On the basis of functional exercise of quadriceps femoris,electroacupuncture and conventional acupuncture can improve the daily activities of KOA patients,and electroacupuncture is better than conventional acupuncture.
