Objective:To observe the effect of electroacupuncture at Zusanli and Shangjuxu combined with conventional western medicine treatment on postoperative ileus(POI)after abdominal surgery,to compare the recovery of the gastrointestinal function in the two groups of patients after treatment,and to investigate its influence on the inflammation and gastrointestinal hormones secretion.So as to provide a certain clinical basis for the future electroacupuncture treatment of POI after abdominal operation.Methods:Sixty patients with postoperative ileus after abdominal surgery admitted to the Gastrointestinal Surgery Department,the Hepatobiliary Surgery Department and the Gynecology Department of the Affiliated Hospital of Jiangxi University of Traditional Chinese Medicine from July 2020 to September 2021 and met the inclusion criteria were selected,and the patients were divided into control group and electroacupuncture group by the random number table,30 cases in each group.The control group received conventional treatment after Western medicine,including monitoring of vital signs,fasting,placing abdominal drainage tube,using gastrointestinal decompression tube for severe symptoms,and also treating with antiinfection,fluid replacement,maintenance of water and electrolyte balance and other symptomatic and supportive treatments.The electroacupuncture group received electroacupuncture on bilateral Zusanli and Shangjuxu on the basis of the control group,which was started 4 days after the operation,the acupuncture was retained for30 minutes,once a day,and the treatment was continued for 4 days.The recovery of gastrointestinal function(the recovery time of bowel sounds,the time of the first exhaust,the time of the first defecation and the normal eating time)were compared between the two groups,the levels of serum inflammatory factors(TNF-α,IL-6,CRP,WBC)and gastrointestinal hormones(MTL,GAS,VIP)were compared on1 st before operation,3rd and 7th after operation,the clinical efficacy of the two groups was compared,and the adverse reactions was observed for safety assessment.Results:1.After treatment,the recovery time of bowel sounds was no statistically significant between the two groups(P>0.05),and the time of the first exhaust and of the first defecation,the normal eating time of the two groups of patients were significantly shorter in the electroacupuncture group than in the control group,and the differences were statistically significant(P<0.05).2.The levels of serum inflammatory factors(TNF-α,IL-6,CRP,WBC)in the two groups at 3rd day after operation were significantly higher than those at 1st day before operation,and the difference was statistically significant(P<0.05).The levels of serum inflammatory factors(TNF-α,IL-6,CRP,WBC)in the group were decreased on the7 th day after operation,and compared with the control group,the decrease was more obvious in the electroacupuncture group(P<0.05).3.The MTL levels of gastrointestinal hormones in the two groups were significantly decreased on the 3rd day after operation,and the difference was statistically significant(P<0.05).In the two groups,the levels of GAS were higher than the 1st day before operation(P>0.05),and the levels of VIP were significantly higher than those 1st day before operation(P<0.05).On the 7th day after operation,both two groups of the MTL and GAS levels were increased,and compared with the control group,the electroacupuncture group increased more significantly,and the difference were statistically significant(P<0.05).And on the 7th day after operation,the levels of VIP in the two groups were decreased,and compared with the control group,the VIP level in the electroacupuncture group was significantly decreased(P<0.05).4.After treatment,the electroacupuncture group’s clinical effective rate was 96.7%,and the control group’s clinical effective rate was 83.3%.The therapeutic effect of the electroacupuncture group was significantly better than that of the control group,and the difference was statistically significant(P<0.05).5.During the electroacupuncture treatment,no obvious adverse reactions occurred in the patients,and it had good safety.Conclusion:On the basis of conventional Western medicine treatment,electroacupuncture at Zusanli and Shangjuxu in the treatment of POI can accelerate gastrointestinal motility,promote the recovery of gastrointestinal function,and effectively relieve the symptoms of POI.The mechanism may be related to that electroacupuncture can reduce the level of inflammatory factors,relieve the inflammatory response of the body,and regulate the release of gastrointestinal hormones.Electroacupuncture has good safety and is worthy of further clinical application.
