| Objective: This clinical study observation aims at observing the clinical efficativeness of acupotomy therapy " modulating sinew to treat bone" in the treatment of knee osteoarthritis,and comparing it with oral etocoxib tablets to objectively evaluate the difference in efficativeness,so as to provide a more reliable evidence for the selection of clinical intervention schemes for knee osteoarthritis.Methods: Sixty patients with knee osteoarthritis who met the standard were randomly divided into experimental group and control group,each group included 30 cases.Acupotomy "modulating sinew to treat bone" method was used in the experimental group,once a week,a whole course of treatment last three weeks.While taking etocoxib tablets was used in the control group,once a day,60 mg each time,the same course with the experimental group.The effectiveness of acupotomy on pain,function and stiffness of patients with knee osteoarthritis was evaluated by Western Ontario and Mc Master University Osteoarthritis Index(WOMAC).The efficativeness of acupotomy on knee passive range of motion(PROM)was measured to analyze the efficativeness of acupotomy on knee passive range of motion.The measurement nodes are set before treatment,one week,three weeks and three months after treatment,and the clinical efficativeness judgement was based on the principles in the Guiding Principles for Clinical Research of New traditional Chinese medicine(Trial).During the treatment,if adverse reactions occurred,time of occurrence,symptoms,treatment measures and treatment results were recorded in detail.Followed up after 3months.The data analysis were runned in IBM SPSS Statistics 24.Result:1.After statistical analysis,no significant difference existed in gender,age,duration of disease,BMI and imaging grade of knee osteoarthritis between the two groups(P>0.05),indicating that comparability existed between the two groups.2.WOMAC score: no significant difference existed between the two groups before treatment(P>0.05).The WOMAC scores of both groups at the time point of one week,three weeks and three months after treatment were lower than those before treatment(P<0.05);The D-value of WOMAC score between the two groups before and after treatment was compared.The D-value of WOMAC score in the experimental group was higher than that in the control group(P<0.05).The average score of WOMAC in the two groups decreased at the time point of one week and three weeks after treatment,and rebounded slightly after three months(P<0.05).The results show that both treatment methods can improve the knee function and state of KOA patients,and the improvement of the experimental group is better than that of the control group.(3)Knee passive range of motion: no significant difference existed between the two groups in knee passive range of motion before treatment(P>0.05).The knee passive range of motion of the two groups at the time point of one week,three weeks and three months after treatment was larger than that before treatment,and the difference was statistically significant(P<0.05).Then compared the D-value of knee passive range of motion between the two groups before and after treatment.The rise range of knee passive range of motion in the experimental group is greater than that in the control group,and the difference is statistically significant(P<0.05).It shows that the effectiveness of KOA in the experimental group on the passive range of motion is better than that in the control group.The average passive range of motion of knee joint in the two groups showed an upward trend after one week and three weeks of treatment,but decreased slightly after three months,and the difference is statistically significant(P<0.05).The results show that the both of the two treatment methods can significantly improve the knee passive range of motion of KOA patients,and the improvement of the experimental group is better than that of the control group.(4)After one course of treatment,the clinical effectiveness of the two groups were compared: in the experimental group,18 cases were significantly effective,11 cases were effective and 1 case was ineffective,with a total effective rate of 93.33%;In the control group,12 cases were markedly effective,14 cases were effective and4 cases were ineffective.The total effective rate was 86.67%.Significant difference existed in the effective rate between the two groups(P<0.05),indicating that the effectiveness of the experimental group on KOA was better than that of the control group.Conclusion: The research shows that the method of "modulating sinew to treat bone" with acupotomy can alleviate symptoms of knee osteoarthritis and improve the passive range of motion of knee joint,with good safety and low recurrence rate,which is worthy of clinical promotion. |
