| Objective:To study the changes of spinal pelvic sagittal parameters before and after posterior lumbar interbody fusion in the treatment of degenerative lumbar spondylolisthesis,and explore the correlation between various parameters and clinical efficacy.Methods:Retrospective analysis of 20 patients who were hospitalized in the First Affiliated Hospital of Nanchang University due to degenerative lumbar spondylolisthesis,accord with the inclusion and exclusion criteria and had complete data and underwent posterior lumbar interbody fusion.There were 3 males and 17 females.According to the preoperative PI angle ≥ 60,45 ≤ PI < 60,the patients were divided into group A and group B.VAS,ODI clinical efficacy indexes and sagittal parameters of spine pelvis were measured before operation and at the last follow-up,The changes of postoperative parameters,VAS and ODI were compared between the two groups.To analyze the correlation between preoperative spinal pelvic sagittal parameters and VAS and ODI and to explore the relationship between postoperative parameters and VAS and ODI.Results:The incision healed well in all patients,and there were no complications such as vascular and nerve injury.VAS score,ODI index,PT,SS and LL in the last follow-up were significantly improved compared with those before operation(P < 0.05).Compared with group A,the ODI score of group B improved more significantly,and the difference was statistically significant(P < 0.05).Pearson correlation analysis showed that preoperative VAS score was correlated with preoperative PI,PTand SS angles(P < 0.05),and ODI score was correlated with preoperative PI,PT,SS and LL angles(P < 0.05).There was correlation between PI,PT and SS before operation(P < 0.05),and there was correlation between LL and PI,SS and SLL.PI,PT,SS and LL in the last follow-up were correlated with ODI score in the last follow-up(P < 0.05).Conclusion:Posterior lumbar interbody fusion is effective in the treatment of degenerative lumbar spondylolisthesis,and can effectively improve the sagittal parameters of spine and pelvis in patients with mild degenerative lumbar spondylolisthesis.PT decreases,SS and LL increase,and SLL changes little after operation.For the treatment of patients with mild degenerative lumbar spondylolisthesis,pay attention to the changes of spinal pelvic sagittal parameters during operation,especially improving Pt angle can obtain better surgical effect.Patients with PI angle ≥ 60 can obtain better surgical effect by completely reducing the spondylolisthesis vertebral body and restoring spinal pelvic parameters during operation. |
