| Objective:A questionnaire on the disease knowledge level of patients with coronary heart disease is compiles to provide an effective assessment tool for medical workers to understand the disease knowledge level of patients with coronary heart disease.Investigate and understand the current situation of the disease knowledge level of patients with coronary heart disease in Gannan Area,and analyze the main factors affecting the disease knowledge level of patients with coronary heart disease,guide them to focus on establishing their own health beliefs and awareness,actively change bad habits,and promote healthy behaviors;the occurrence and development of coronary heart disease,improve the quality of life,and guide the active learning of coronary heart disease-related health knowledge,help medical workers pay attention to coronary heart diseasehealth education,and providea basis for reducing the overall cardiovasculardisease incidencein Gannan Area.Methods:1.Delphi expert correspondence and reliability and validity test:A total of 20correspondence experts were selected from cardiovascular clinical medical experts,cardiovascular clinical nursing experts and psychopsychology experts.Through two rounds of expert letter inquiries,the initial“Questionnaire on the Knowledge Level of Coronary Heart Disease Patients”was formed;using the initially formed questionnaire,248 patients with coronary heart disease in the cardiovascular department of a third-class hospital in Gannan Area were selected for preliminary survey and the survey results were sorted out,using Items were screened by item analysis,factoranalysis,etc.Afterscreening,thereliability and validity ofthe questionnaire was tested,and a formal questionnaire survey was conducted on 225 patients with coronary heart disease.Confirmatory factor analysis was used to verify the structural fit of thequestionnaire,and the final questionnaire was formed.2.Coronary heart disease patients survey and analysis of influencing factors:A total of473 coronary heart disease patients in the cardiovascular department of a third-class hospital in Gannan Area were selected as the survey objects by the convenience sampling method for questionnaire survey.The questionnaires included a self-designed questionnaire on the general demographic data of patients with coronary heart disease and a questionnaire on the level of knowledge of patients with coronary heart disease.The current status and influencing factors of thesurvey results wereanalyzed.3.Statistical methods:WPS-Excel、SPSS 23.0 and AMOS 26.0 were used for data statistics and analysis,count data were described by frequency and percentage(%),and measurement data were analyzed by mean±standard deviation(X±S)description;two independent samples t test、one-way ANOVA、nonparametric test、and multiple linear regression analysis were used to understand the influencing factors of the level of disease knowledge inpatients with coronary heart disease.Results:1.Delphi expert correspondenceandreliability and validity test results:The positive coefficients of the two rounds of expert letter inquiries were both 100%,and therates of expert opinions were25%and 0%respectively;the arithmetic meanof the authority coefficients(Cr)were:0.87、0.89;the arithmeticmean of the coefficientof variation(Cv)in the degree of coordination values:0.13、0.11;Kendall’s Harmony Coefficient(W):0.24、0.09;P values areall less than 0.05.Item analysis and factor analysis were carried out according to the survey results of the initial test sample,and a total of 8 items were deleted.The overall Cronbach,s alphacoefficient of the questionnaire was 0.92,and the half coefficient was 0.88;the Cronbach,s alpha coefficients of each dimension were 0.84、0.85、0.83、0.83、0.82 respectively.The range of I-CVI ofthe questionnairewas 0.80-1.00,andthe S-CVI was 0.97;exploratory factoranalysis showed that the KMO value of the questionnaire was 0.89,Bartlett,s sphericity test P<0.01,and the cumulative variance contribution rate was 62.18%,forming a total of 5 factors.The correlation coefficient between each item and the total score ranged from 0.49-0.67,and the correlation coefficient between each factorand thetotal score ranged from 0.70-0.83;the results ofconfirmatory factoranalysis showed that the model fit was good,the X~2/DFvalue was 2.39,P<0.001,RMSEAThe value was 0.08,the TLI valuewas 0.90,the IFI valuewas 0.92,and the CFI value was 0.92.Atotal of 25 items were finally determined in the questionnaire on the knowledge level of coronary heart disease patients:6 items of perceived disease severity,6items of perceived disease susceptibility,5 items of perceived behavioral disorder,4 items of perceivedbehavioral effectiveness and 4 items of self-efficacy entry.2.Status quo of the diseaseknowledgelevel of patients with coronary heart disease:(1)A questionnaire survey was conducted on 473 patients in Gannan Area by using the general demographic data questionnaire and the coronary heart disease patient disease knowledge level questionnaire,the total score was(88.56±17.59)points,items weredivided into(3.54±1.08)points,theresults showed a middle level;thehighest scoring dimension was perceived disease severity(22.66±4.78)points,and the lowest scoring dimension was perceivedbehavioral effectiveness(14.14±3.41)points.(2)The results of univariate analysis showed that there were statistically significant differences in age、marital status、educational level、living style、family location、occupation、income、payment method ofmedical expenses、years of illness、family history and knowledge sources(P<0.05);there was no significant difference in gender and other chronic diseases(P>0.05).(3)The results of multiple linear regression analysis showed that the main factors affecting thelevel of disease knowledgein patients with coronary heart diseaseincluded:education level(B=2.87,P=0.000)、income(B=5.46,P=0.000)、years of illness(B=2.42,P=0.000)、knowledge sources(B=-1.96,P=0.003).Conclusion:The questionnaire on the knowledge level of coronary heart disease patients prepared in this study has good reliability and validity,and can be used to evaluate the disease knowledge level of patients with coronary heart disease in Gannan Area.The disease knowledge level of patients with coronary heart disease in Gannan Area is at a medium level,indicating that residents in Gannan Area have little knowledge of coronary heart disease,and the knowledge level needs to be improved.Through the analysis of influencing factors,it is suggesting that relevant medical institutions should carry out to target clinical intervention measures and publicize health education content for patients with coronary heart disease,so as to provide better medical care services,reduce hospitalization rate,improve prognosis and quality of life;also pay attention to high-risk patients The screening and physical examination of the population guide coronary heart disease patients and high-risk groups to focus on establishing their own health beliefs and awareness,take the initiative to change bad habits,promote healthy behaviors,and protect thehealth ofresidents in Gannan Area as much as possible. |
