Objective:To compare the efficacy and safety of intelligent controlled pressure Flexible and Rigid ureteroscopic lithotripsy with Mini-percutaneous nephrolithotomy for the treatment of staghorn calculi,explore the advantages of intelligent controlled pressure Flexible and Rigid ureteroscopic lithotripsy in the treatment of staghorn calculi.Methods: Retrospective analysis was performed on the clinical data of patients with staghorn calculi admitted to the Department of Urology of People’s Hospital from October 2019 to October 2021.A total of 83 patients were included in the study,and they were divided into intelligent pressure-controlled flexible and rigid ureteroscopy group and mini-percutaneous nephroscopy group according to surgical methods.Independent sample Student’s test and Chi-square test were used to compare the operative success rate,stone clearance rate,complication rate,mean operation time,mean lithotriptic time,length of stay and hemoglobin drop between the two surgeries.Results: The average time of lithotripsy was(151.56±20.10)min and(119.21±21.76)min and the average hem oglobin loss was(6.21±3.62)g/L and(13.74±8.67)g/L,and the average length of stay was(3.45±0.66)days and(7.06±2.60)days in the intelligent controlled pressure ureteroscopy group and the mini-percutaneous nephroscopy group,respectively,whose differences were statistically significant(P < 0.05).However,there was no significant difference in the mean operation time between the two groups((178.28±42.76)min vs(164.77±23.74)min,P=0.076).There was no significant difference in the incidence of postoperative complications such as postoperative fever,sepsis,blood transfusion,interventional embolization and pleural effusion between the two groups(P > 0.05),but the overall incidence of complications in the flexible ureteroscopy group was lower than that in the percutaneous nephroscopy group(12.5%vs31.9%,P < 0.05).The stage 1 stone-free rates in flexible and rigid ureteroscopy group was lower than that in mini-percutaneous nephroscopy group(68.8%vs87.2%,P < 0.05),but there was no significant difference in overall stone-free rates and times of stone clearing between the two groups(P > 0.05).Conclusions: Intelligent pressure controlled transurethral Flexible and Rigid ureteroscopic lithotripsy for staghorn calculi is effective and safe,which can achieve the effect of Mini-percutaneous nephrolithotomy for a certain number of times,and provide another surgical method for both doctors and patients.
