Objective:The levels of serum uric acid(UA),red blood cell distribution width(RDW)and mean platelet volume(MPV)were measured in elderly patients with different grades of cardiac function to investigate the correlation between serum UA,RDW and MPV levels and cardiac function in elderly patients with chronic heart failure.Methods:A total of 159 elderly patients hospitalized in the Second Hospital of Shanxi Medical University from January 2020 to December 2021 were retrospectively analyzed,of which 111 elderly patients diagnosed with chronic heart failure were the case group.The case group was divided into group A(NYHA class II,31 cases),group B(NYHA class III,37 cases)and group C(NYHA class IV,43 cases)according to NYHA cardiac function classification.Elderly patients hospitalized for hypertension and arrhythmia without clinical manifestations of heart failure were 48 as the control group.The levels of UA,MPV and RDW of the study subjects in each group were compared and analyzed,Pearson analysis was applied to analyze the correlation between the parameters,logistic analysis was applied to analyze the risk factors of chronic heart failure in the elderly,and the subject operating characteristic curve(ROC)was used to evaluate the predictive value.Results:Compares with the control group,patients in the case group has significantly higher levels of UA,RDW,MPV and BNP,and the levels of UA,RDW,MPV and BNP increased with increasing cardiac function class,with statistically significant differences(P < 0.05);Serum UA,RDW and MPV were positively correlated with BNP and LVEDD(r=0.456 and 0.268,0.051 and 0.229,0.237 and 0.217,p<0.05)and negatively correlated with LVEF(r=-0.356,-0.412,-0.209,p<0.05).The value of RDW in predicting the severity of cardiac function in elderly CHF is higher than that of UA and MPV,and the predictive value of MPV was the lowest,with the area under the curve The area was:0.87,0.781 and 0.66.When RDW was 45.6 f L,the sensitivity was 73.9% and specificity was 89.6%;when UA was 365.3 umol/L,the sensitivity was 77.5% and specificity was66.7%;when MPV was 9.85 f L,the sensitivity was 80.2% and specificity was 52.1%.Conclusion:UA,RDW and MPV levels at admission are clearly correlated with the cardiac function class of elderly CHF patients,and UA,RDW and MPV can be used to clinically assess the progression and prognosis of elderly patients with chronic heart failure.
