Purpose:This study mainly collected medical records of hospitalized patients in the Cardiovascular Department of Dongfang Hospital,Beijing University of Traditional Chinese Medicine,and recorded the hemoglobin,standard deviation of red blood cell distribution width(RDW-SD),coefficient of variation of red blood cell distribution width(RDW-CV),N-terminal brain natriuretic peptide(NT proBNP),left ventricular ejection fraction(LVEF),left ventricular end diastolic diameter(LVEDD),and NYHA cardiac function grading level of patients with chronic heart failure(CHF),Explore the relationship between various indicators and traditional Chinese medicine syndrome differentiation and classification,in order to provide reference for the TCM syndrome differentiation and clinical diagnosis and treatment of CHF.Method:According to the inclusion and exclusion criteria,case data of 171 patients were collected,including:(1)General information of patients:name,medical record number,gender,age,height,weight,comorbidities,etc.(2)Western medicine observation indicators:including blood routine indicators such as hemoglobin(HGB),standard deviation of red blood cell distribution width(RDW-SD),coefficient of variation of red blood cell distribution width(RDW-CV),NT proBNP level,left ventricular ejection fraction(LVEF),left ventricular end diastolic diameter(LVEDD),NYHA heart function grading level,as well as biochemical indicators such as triglycerides(TG),total cholesterol(TC)Low density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),serum creatinine(Scr),uric acid(UA),fasting blood glucose(FBG),homocysteine(Hcy),aspartate aminotransferase(AST),alanine aminotransferase(ALT),etc.(3)Traditional Chinese Medicine Four Diagnosis Materials:Traditional Chinese Medicine Symptoms,Signs,Tongue Images,Pulse Images,and Traditional Chinese Medicine Syndrome Differentiation and Typing.Statistical data analysis application software SPSS25.0 completed.Result:(1)The NYHA cardiac function grading distribution of patients with chronic heart failure showed significant differences in different traditional Chinese medicine syndromes(P<0.01).(2)There was no statistically significant difference in age distribution among patients with different NYHA cardiac function grades(P>0.05),and there was no statistically significant difference in gender composition between groups(P>0.05).There was no statistically significant difference in age distribution among different groups of patients with different TCM syndrome types(P>0.05);There was no statistically significant difference in gender composition between the groups(P>0.05).(3)There were statistical differences in the levels of Scr,UA,TC,LDL-C,HDL-C,and Hcy among patients with different NYHA cardiac function grades(P<0.05);There was no statistically significant difference in ALT,AST,TG,and FBG levels(P>0.05).There are statistical differences(P<0.05)in the levels of Scr,UA,TC,and HDL-C among patients with different traditional Chinese medicine syndrome types;There was no statistically significant difference in the levels of ALT,AST,TG,LDL-C,FBG,and Hcy(P>0.05).(4)There were significant statistical differences(P<0.01)in the levels of HGB,RDW-SD,RDW-CV,and NT proBNP among patients with different NYHA cardiac function grades.(5)There were significant statistical differences(P<0.01)in the levels of HGB,RDW-SD,RDW-CV,and NT proBNP among patients with different traditional Chinese medicine syndrome types.(6)The HGB level in CHF patients is negatively correlated with NT proBNP and LVEF levels,and positively correlated with LVEDD levels.The levels of RDW-SD and RDW-CV are positively correlated with NT proBNP levels,but not with LVEF and LVEDD levels.(7)LVEF level is negatively correlated with NYHA cardiac function grading and different traditional Chinese medicine syndrome types;LVEDD and NT proBNP levels are positively correlated with NYHA cardiac function grading and different traditional Chinese medicine syndrome types.(8)The HGB level is negatively correlated with NYHA cardiac function grading and different traditional Chinese medicine syndrome types;The levels of RDW-SD and RDW-CV are positively correlated with NYHA cardiac function grading and different traditional Chinese medicine syndrome types.Conclusion:(1)There may be a certain correlation between the traditional Chinese medicine syndrome types and the level of cardiac function grading in CHF patients.(2)The HGB level of CHF patients may have a certain correlation with their NYHA cardiac function grading and traditional Chinese medicine syndrome types,suggesting that HGB may serve as a reference for clinical guidance of traditional Chinese medicine syndrome differentiation and classification in CHF patients.(3)The levels of RDW-SD and RDW-CV in CHF patients may have a certain correlation with their NYHA cardiac function grading and traditional Chinese medicine syndrome types,suggesting that RDW may serve as a reference for clinical guidance of traditional Chinese medicine syndrome differentiation and classification in CHF patients.
