| Objective:1.To evaluate the level of serum Asprosin in patients with type 2 diabetes and explore its correlation with carotid artery intima-media thickness.2.To explore the value of serum Asprosin in the diagnosis of carotid atherosclerosis.Methods:1.According to the inclusion and exclusion criteria,104 type 2 diabetes mellitus(T2DM)patients who were hospitalized in the Endocrinology Department of our hospital between December 2020 and December 2021 were selected.According to the results of carotid intima-media thickness(CIMT)measured by carotid ultrasound,they were divided into two groups.3 groups: no carotid atherosclerosis group(D group,n=43),carotid atherosclerosis group(L group,n=30),carotid atherosclerosis with plaque formation group(H group,n=31);Healthy people in the physical examination center of our hospital served as the control group(group C,n=32).2.Collect and record the general information and blood biochemical indexes of all participants,and measure the serum Asprosin level of all participants by enzyme-linked immunosorbent assay.3.Compare the differences in clinical indicators and serum Asprosin levels among the groups;divide the patients with type 2 diabetes into two groups according to gender,BMI,and whether combined with MAFLD or not,and then compare whether there are differences in serum Asprosin levels;analyze the correlation between CIMT and each index and its influencing factors;draw ROC curve to evaluate the diagnostic value of Asprosin for carotid atherosclerosis.Results:1.There was no significant difference in gender,age,SBP,DBP,TC,HDL-C,FINS,FC-P among the groups(P>0.05).The differences among Hb A1 c,FPG,HOMA-β,and HOMA-IR were statistically significant(P<0.05).2.Compared with group C(1.11±0.17ng/m L),group D(1.89±0.76ng/m L),group L(2.22±0.58ng/m L)and group H(2.58±0.86ng/m L)had serum asprosin The level of serum asprosin in group H was significantly higher than that in group D,and the difference was statistically significant(P<0.05).There was no significant difference in serum Asprosin levels between L groups(P>0.05).3.The T2 DM patients were grouped according to gender,and there was no significant difference in serum Asprosin levels between males and females(P>0.05).According to the BMI grouping,the serum Asprosin level in the obese group was significantly higher than that in the non-obese group,and the difference was statistically significant(P<0.05).The T2 DM patients were divided into T2 DM with or without MAFLD group according to whether they were combined with MAFLD.It was found that the serum asprosin level in the T2 DM with MAFLD group was significantly increased,and the difference was statistically significant(P<0.05).4.After Spearman correlation analysis of all T2 DM patients,CIMT in T2 DM patients was negatively correlated with HOMA-β(r=-0.200,P=0.042),and was negatively correlated with diabetes duration(r=0.220,P=0.025),MAFLD(r= 0.338,P<0.001),FPG(r=0.205,P=0.037),and serum Asprosin level(r=0.360,P<0.001)were positively correlated.5.The results of ordered multi-class logistic regression analysis showed that the duration of diabetes(β=0.135,P<0.001),FPG(β=0.179,P=0.013),MAFLD(β=1.253,P<0.001),serum Asprosin level(β=1.278,P<0.001)was an independent influencing factor of CIMT in T2 DM patients.6.ROC curve analysis showed that the area under the ROC curve(AUC)of serum Asprosin level for the diagnosis of T2 DM complicated with carotid atherosclerosis was0.688,P=0.002,95% CI(0.576,0.800),and the best cut-off value was 2.31ng/m L,the sensitivity was 0.645,and the specificity was 0.712,suggesting that Asprosin has a certain diagnostic value for carotid atherosclerosis.Conclusion:1.The serum Asprosin level in each group of T2 DM patients was significantly higher than that in the healthy control group,and its serological level increased gradually with the thickening of CIMT.Correlation analysis showed that Asprosin level was positively correlated with CIMT.Logistic regression analysis showed that Asprosin was an independent influencing factor of carotid atherosclerosis,suggesting that Asprosin was involved in the occurrence and development of diabetic macrovascular disease.2.The duration of diabetes,FPG,MAFLD were positively correlated with CIMT,which were independent factors of CIMT,and were closely related to the occurrence of diabetic macrovascular disease.3.Serum Asprosin has certain diagnostic value for carotid atherosclerosis,and can be used as an effective serological indicator for the diagnosis of carotid atherosclerosis. |
