| Objective:By performing the determination of rheumatoid arthritis(Rheumatoid arthritis,RA)Level of Th,Th1,Th2,Th17,Treg cells,osteopontin(OPN),and tenosin-C(TN-C)in the peripheral blood of primary and refractory patients,To investigate the clinical significance of Th,Th1,Th2,Th17 and Treg cell number levels,OPN concentration and TN-C concentration in the peripheral blood of patients with naive and refractory RA,To analyze the correlation in naive and refractory patients with RA,Looking for the possible pathogenesis of RA,Providing potential therapeutic targets.Methods:A total of 36 patients diagnosed for RA in our department from January 2019 to January 2022:23 female,13 males,13-81 years,average(55.25 ± 14.86),53 patients with refractory RA: 35 women,18 males,24-84 years,average(57.11 ± 12.13)were selected.At the same time,30 healthy physical examination patients in the same period were selected as the normal control group(HC group),including 22 women,8 males,20-69 years old,the average(48.07 ± 12.35).Three fasting venous blood tubes were drawn in the morning,One tube of blood was measured in Th,Th1,Th2,Th17,and Treg cells by flow cytometry,The other two tubes of blood were measured for OPN by enzyme-linked immunosorbent assay(ELISA),And collected the clinical data of the RA patients,These include gender,age,joint swelling,number of tenderness,Laboratory indicators: blood sedimentation rate(ESR),C reactive protein(CRP),rheumatoid factor(RF),anti-cyclic citrulline peptide antibody(Anti-CCP),etc.,The DAS28 scores associated with disease activity were calculated for patients with RA.The levels of Th,Th1,Th2,Th17,Treg,OPN and TN-C were compared in the peripheral blood of the three groups;the correlation between OPN,TN-C and Th,Th1,Th2,Th17,Treg and DAS28 in the primary and refractory RA groups was analyzed.Results:1.Levels of OPN,TN-C,Th,Th1,Th2,Th17 and Treg cells in the peripheral blood of HC,primary and refractory groups: the number of Th cells(748.29 ± 236.94)in H C group / ul,Number of Treg cells(39.56 ± 13.46)/ ul;The number of Th cells(664.50 ±317.04)/ ul,Number of Treg cells(32.64 ± 30.56)/ ul;Number of refractory Th cells(752.87 ± 342.04)cells / ul,Number of Treg cells(32.20 ± 18.79)cells / ul;Comparison of the number of Th and Treg cells between the three groups,The difference was not significant(P=0.379;P=0.306).Th1,Th1,Th17,OPN,and TN-C,Th1(P=0.03),OPN(P=0.003),TN-C(P <0.001),Th1(P <0.05),OPN(P> 0.05),TN-C(P <0.01),T h 1(P<0.05),OPN and TN-C(P <0.01;P <0.01).2.Correlation analysis of clinical data of the primary treatment group and cell number levels of OPN,TN-C and Th,T h,Th1,Th2,Th17,T and Treg in peripheral blood: significant positive correlation between OPN and TN-C(P <0.01),A positive correlation with the DAS28 score(P <0.05);TN-C was only significantly and positively associated with OPN;Treg and Th cell number related positively(P <0.01);Th and number of Th1 and Th2 cells(P <0.01;P<0.01),Positive correlation with the number of Th17 cells(P <0.05);Th1 and Th2 and Th17 cells(P <0.01;P<0.01),A negative correlation with DAS28 score and number of tenderness(P <0.05;P<0.05);Th17 cell number was negatively correlated with DAS28 score and number of tenderness(P <0.05;P<0.05).3.Correlation analysis of clinical data of the refractory group and cell number levels of OPN,TN-C and Th,T h,Th1,Th2,Th17 and Treg in peripheral blood: OPN and TN-C also showed a significant positive correlation(P <0.01),Negative correlation with the number of Th17 cells(P <0.05);TN-C also only showed a significant and positive correlation with OPN;Treg and Th cell number related positively(P <0.01);Th was positively associated with the number of Th1 and Th2 cells(P <0.05;P<0.05),Negative correlation with the DAS28 score(P <0.05);Th1 and Th2 cell number(P <0.01);Significant positive correlation between Th2 and Th17 cell number(P <0.01);The Th17 cell number was inversely associated with the DAS28 score(P <0.05).Conclusion:1.OPN concentration in naive RA patients was not significantly but was positively associated with disease activity;TN-C concentration increased but not with disease activity;Th1 cell number decreased and negatively correlated with disease activity.2.OPN and TN-C concentrations in refractory RA patients increased,but not with disease activity;Th1 cell number decreased,but not with disease activity.3.TN-C concentration was increased in naive RA and refractory RA patients,OPN increased in naive RA patients,OPN increased in refractory RA patients,and OPN and TN-C concentrations showed a significant positive correlation between the two groups,indicating that OPN and TN-C concentrations have certain significance in the diagnosis of RA patients. |
