Objective:Guided by evidence-based theory,an intervention plan for discharge readiness of stroke patients was constructed,and its intervention effect was evaluated.Methods:1.Intervention program construction stage: Retrieve relevant guidelines,expert consensus and literature on the readiness of stroke patients for hospital discharge,and use Appraisal of Guidelines for Research and Evaluation II(AGREE II)to evaluate the quality of the guidelines The literature was evaluated with the literature evaluation standards of the Australian Evidence-Based Health Care Center(Joanna Briggs Institute,JBI).After extracting evidence items,the JBI evidence pre-grading system was used to grade the evidence.After group discussion,the first draft of the intervention plan was formulated,and two rounds of experts were implemented.After the letter inquiries,according to the scores and opinions of experts,the items were revised and improved after group discussion to form the final intervention plan.2.The implementation stage of the intervention plan: The intervention plan constructed in the previous stage was evaluated by quasi-experimental research methods,and 92 brain patients who were hospitalized in the first and second wards of the Department of Neurology,the First Hospital of Shanxi Medical University from September to October 2021 were selected.Stroke patients were taken as the research objects,the first ward was the experimental group(n=46),and the second ward was the control group(n=46).Nursing,medication nursing,psychological nursing and complication prevention,etc.,the experimental group implemented the hospital discharge readiness intervention program on the basis of routine nursing.Before the intervention,the general data of the patients were collected,and after the intervention,the readiness for hospital discharge and the quality of discharge guidance were collected 4 hours before discharge,and the readmission rate 1month after discharge was counted.Results:1.Intervention program construction stage: In the two rounds of correspondence,the positive coefficients of experts were 100%,the coefficients of expert authority were 0.81 and 0.84,and the importance of each item Kendall’s harmony coefficients were 0.165 and 0.453,respectively,and the differences were statistically significant(P<0.001).The intervention plan constructed in this study includes "assessing risk factors and conducting timely monitoring","strengthening basic care and guarding against stroke recurrence","defining rehabilitation goals and formulating training plans","active health education and improving nursing ability","providing learning Resources,establish health records" and "consolidate self-care knowledge,do a good job of scale evaluation" 6 first-level items and 23second-level items.2.The implementation stage of the intervention plan: 4 hours before discharge,the total score of hospital discharge readiness and the scores of each dimension in the experimental group were higher than those in the control group,and there was a statistical difference between the scores of the two groups of patients(P<0.05);4hours before discharge,the experimental group was discharged from the hospital The scores of guidance quality,content dimensions actually obtained before discharge,content difference dimensions,discharge guidance skills and effect dimensions were all higher than those of the control group,and there was a statistically significant difference between the scores of the two groups(P<0.05).There was no significant difference in dimensions(P>0.05);there was a statistically significant difference in the unplanned readmission between the two groups at 1 month after the intervention(P<0.05),and the unplanned readmission rate in the control group was higher than that in the experiment Group.Conclusion:1.By retrieving relevant guidelines,consensuses,and literature on the readiness of stroke patients for discharge,and evaluating the quality,summarizing relevant evidence and evaluating its level,implementing two rounds of expert correspondence,and revising the entries after group discussion according to expert scores and opinions and improvement,forming an intervention plan for stroke patients’ discharge readiness including 6 first-level items and 23 second-level items.2.The experimental study method was used to verify the effect of the implementation of the plan.Compared with the control group,the total score of discharge readiness and its various dimension scores,the quality of discharge guidance and its various dimension scores,and the non-discharge rate within 1month after discharge in the experimental group were verified.There has been a marked increase in planned readmission rates.It shows that the implementation of this research scheme has improved the discharge readiness and the quality of discharge guidance for stroke patients,and provided a basis for clinical medical staff to carry out relevant interventions. |
