Purpose of researchCollect the clinical data of patients with coronary heart disease complicated with pre-diabetes,and study the internal correlation with"symptom-syndrome-physicochemical indexes"as the pointcut so as to provide more clinical ideas for TCM syndrome differentiation.Material and methodAccording to medical records inclusion standards,a total of 105 patients with coronary heart disease complicated with pre-diabetes who were hospitalized in the First Affiliated Hospital of Heilongjiang University of Traditional Chinese Medicine and Traditional Chinese Medicine from November 2020 to December 2021 were included,and clinical data of the included patients were collected.Data mining:SPSS 26.0 statistical software and Origin 8.6 software were used to collate data,chi-square test,analysis of variance and non-parametric test were used to analyze general data,systematic clustering,factor analysis and association rules were used to analyze the characteristics of TCM syndrome elements,and Bayesian network in Ge NIe2.3 software was used to analyze the syndrome rule.The value of physical and chemical indexes in syndrome differentiation was analyzed based on ROC curve,and its sensitivity and specificity were calculated.Results1.General Information:there were statistically significant differences in different age groups,courses of coronary heart disease and BMI(P<0.05).In the age group of 50-59 years,10-19 years of coronary heart disease course,BMI>24kg/m~2cases were the most;There were statistically significant differences in WC and WHR between different genders(P<0.05),and male patients had higher WC and WHR.Among the other comorbidities,one comorbidities accounted for 39.04%,among which,concurrent hypertension ranked first at 16.19%.2.TCM syndrome elements:according to the data table of factor score,a total of 19 syndrome elements variables were obtained in 105 patients,the main pathogenic elements included:yin deficiency(42),heat(41),qi deficiency(29),blood deficiency(29),qi stagnation(22),dampness(14);The main factors of disease location elements included:heart(63),spleen(60),kidney(37).3.TCM syndrome type:according to the results of systematic clustering of syndrome elements and combination of syndrome elements,four syndrome types of coronary heart disease with pre-diabetes were obtained:qi-yin deficiency pattern(34.71%),pattern of dampness-heat in the spleen and stomach(31.18%),pattern of qi stagnant with blood stasis(17.65%),pattern of yang deficiency with water retention(16.47%).4.Comparison of risk factors related to different syndrome types:there were no statistically significant differences in gender and other medical history among different TCM syndrome types(P>0.05),while there were statistically significant differences in age,courses of coronary heart disease,BMI,WC and WHR among TCM syndrome types(P<0.05).After pair-wise comparison,the age and course of coronary heart disease of qi-yin deficiency pattern and pattern of yang deficiency with water retention were higher(P<0.05),and the BMI,WC and WHR in pattern of dampness-heat in the spleen and stomach and pattern of yang deficiency with water retention were higher(P<0.05).5.Comparison of physical and chemical indexes of different syndrome types:High LDL-C was more common in the qi-yin deficiency pattern(P<0.05);High TG(P<0.05)and high HDL-C(P<0.05)was more common in the pattern of dampness-heat in the spleen and stomach;High PDW(P<0.05),high MPV(P<0.05),high FIB(P<0.05)and low PT were the most common in the pattern of qi stagnant with blood stasis(P<0.05);High UREA(P<0.05),high CREA(P<0.05),high blood sodium(P<0.05)and low albumin(P<0.05)were the most common in pattern of yang deficiency with water retention.6.The sensitivity and specificity of physical and chemical indexes in syndrome type were calculated by ROC curve:In the qi-yin deficiency pattern,LDL-C(66.7%,81.2%);In the pattern of dampness-heat in the spleen and stomach:TG(78.6%,59.7%),HDL-C(60.7%,75.3%);In the pattern of qi stagnant with blood stasis:Distribution of PDW(68.2%,90.4%),MPV(90.9%,54.2%),FIB(68.2%,73.5%);In the pattern of yang deficiency with water retention,blood sodium(57.9%,93.0%),CREA(73.7%,74.4%),UREA(52.6%,98.8%).Research conclusion1.Coronary heart disease complicated with pre-diabetes was correlated with age,courses of coronary heart disease,BMI.The patients aged 50-59years and the courses of coronary heart disease was 10-19 years,BMI>24kg/m~2accounted for the most.WC and WHR were different in different genders,male patients had higher WC and WHR.2.There was one comorbidities accounted for the most,in which hypertension ranked first.3.The main pathogenic elements of coronary heart disease complicated with pre-diabetes included:yin deficiency,heat,qi deficiency,blood deficiency,qi stagnation,dampness;The main disease location elements include heart,spleen and kidney.3.Four TCM syndromes of coronary heart disease complicated with pre-diabetes:qi-yin deficiency pattern,pattern of dampness-heat in the spleen and stomach,pattern of qi stagnant with blood stasis,pattern of yang deficiency with water retention.4.Relationship between TCM syndrome type and related risk factors:Gender,history of other no correlation between the TCM syndrome types,age,courses of coronary heart disease,BMI,WC,WHR in a correlation within different TCM syndrome types,among them with qi-yin deficiency pattern and pattern of yang deficiency with water retention performed at a higher age and courses of coronary heart disease,pattern of dampness-heat in the spleen and stomach and pattern of yang deficiency with water retention showed a higher BMI,WC,WHR.5.Valuable physical and chemical indicators of sensitivity and specificity in syndrome differentiation of coronary heart disease complicated with pre-diabetes:qi-yin deficiency pattern:LDL-C;Pattern of dampness-heat in the spleen and stomach:TG,HDL-C;Pattern of qi stagnant with blood stasis:PDW,MPV,FIB;Pattern of yang deficiency with water retention:blood sodium,CREA,UREA.
