Research purpose:This study aims to explore and describe the medical barriers and needs of cancer patients in our country.Based on the iceberg model and nursing procedures,a localized oncology nurse navigator access standard and job responsibilities plan are constructed,so as to provide a standardized development of patient navigation in the future.Patterns provide a reference.Materials and Methods:A descriptive research method was used to select 12 cancer patients and 15 patients’ family members from December 2020 to March 2021 in a tertiary grade A tumor hospital in Sichuan Province by purpose sampling method for semi-structured interviews.Content analysis method was used.Organize and analyze interview data.Combined with the interview results,through literature analysis,group discussion and pre-consultation,the entry standards and work responsibilities plan of cancer nurse navigators were initially constructed.By using the Delphi method,22 experts from 11 provinces and cities were used to conduct questionnaire consultation,using AHP.method to determine the indicator weight.Data entry and statistical analysis were performed using Excel 2013 and SPSS24.0.Result:1.Qualitative research results.Barriers to medical treatment for cancer patients can be divided into two themes: individual factors(lack of information,cognitive misunderstandings,poor psychological state);environmental factors(insufficient social support,insufficient access to medical treatment,inadequate continuous medical services,and emergencies in public health events);medical needs of cancer patients: psychological needs,social needs,knowledge needs.2.Results of expert consultation and weighting.Respectively,the effective recovery rates of the two expert consultations were 91% and 100%.In the second round,the expert authority factor was 0.875,and Kendall’s W of access criteria and work duties were 0.164,0.303,0.15,0.203,and 0.238,respectively(all P <0.05).The final admission standard and job responsibility plan for oncology nurse navigators included 4 first-level indicators and22 second-level indicators of the admission standard;4 first-level indicators,15 secondlevel indicators,and 49 third-level indicators for job duties.Among the secondary indicators of the access standard,the highest weight is the communication and coordination ability(0.169).Among the first-,second-,and third-level indicators of job duties,the highest weights were implementation(0.394),implementation communication(0.183)and evaluation of patient compliance(0.081).Conclution:1.Chinese cancer patients have many difficulties in seeking medical treatment,which are affected by many personal and environmental factors.Managers should formulate targeted interventions according to the importance level of each factor,so as to promote patients to seek medical treatment in a timely manner and improve cancer health outcomes.2.The admission standards and job responsibilities of oncology nurse navigators determined by expert consultation are scientific,feasible,practical and innovative,and can provide theoretical basis for the development of patient navigation and the training of oncology nurse navigators in our country.
