| Objective:The medical records of the patients with Hashimoto’s thyroiditis and nodules were collected in the clinic,and the medication rules of Professor Ma Jian in the treatment of Hashimoto’s thyroiditis and nodules were analyzed,and his experience in the treatment of Hashimoto’s thyroiditis and nodules was summarized.and academic thinking.Method:The Patients’ medical record,which visited Professor Ma Jian ’s Outpatient department of Endocrinology in the First Affiliated Hospital of Heilongjiang University of Chinese Medicine from January 2017 to December2020,were selected.According to inclusion and exclusion criteria,234 eligible prescriptions were screened out.Then,statistical analysis,prescription analysis,symptom analysis and tongue and pulse analysis were carried out by TCM inheritance and development platform V3.0.To summarize professor Ma Jian ’s experience and academic thought in the treatment of Hashimoto with nodules.Results:1.Among the 234 medical records of Hashimoto’s thyroiditis complicated with nodules,the male to female ratio was 1:13.62.The age distribution is30-60 years old,accounting for 83.76%.2.There were 21 kinds of tongue images in the 234 medical cases,202 times of red tongue,189 times of white coating,141 times of dentate tongue and 59 times of fat tongue,accounting for 88.22% of the total.3.The 234 medical cases included 16 types of pulse patterns,77 times of slippery pulse,73 times of slippery pulse,and 30 times of slippery pulse,accounting for 76.60% of the total.4.A total of 291 kinds of symptoms were counted in 234 cases,of which the symptoms with frequency greater than 15 were thyroid nodules,fatigue,insomnia,shortness of breath,chest tightness,diarrhea,frequent dreams,oligomenorrhea,tachypnea,palpitations,irritability and ease.Anger,dry mouth,dizziness.5.There are 40 drugs with a frequency of more than 19 drugs,accounting for 88.02% of the total.6.Analysis of the four qi,five flavors and the meridian of traditional Chinese medicine: the warm,cold and flat drugs in the four qi accounted for94.08%;the bitter,pungent and sweet drugs in the five flavors accounted for93.48%;the drugs entered the liver meridian in the meridian,spleen meridian,lung meridian,heart meridian and stomach meridian accounted for 80.12%.7.Efficacy types of traditional Chinese medicine: the top six in terms of frequency of use are:heat-clearing,blood-activating,blood-stasis-removing,phlegm-relieving,cough-relieving,asthma-relieving,deficiency-tonifying,qi-regulating,water-dampening and dampness-relieving.8.The core combination of drugs and core prescriptions: Zhe Fritillaria and Platycodon grandiflorum,tangerine peel and Zhe Fritillaria,tangerine peel,Platycodon grandiflorum and tangerine peel,Platycodon and Zhe Fritillaria,tangerine peel and Angelica;7 core prescriptions were obtained through cluster analysis.Conclusion:1.Professor Ma Jian’s medicines for Hashimoto’s thyroiditis complicated with nodules are mostly warm and cold medicines.The five flavors are often bitter,acrid,and sweet,and they return to the liver,spleen,lung and heart meridian,reflecting heat-clearing,blood-activating,blood-stasis-removing,and phlegm-removing.The power of softening and dispersing knots,together with soothing the liver and regulating qi,invigorating the spleen and benefiting the lungs,etc.,in the process of diagnosis and treatment,focus on seeking the root of the disease,and applying both attack and supplement.2.Professor Ma Jian’s treatment of Hashimoto’s thyroiditis complicated with nodules is often treated with the self-made prescription Xuanxia Xiaoying Decoction.The core drugs are tangerine peel,fritillaria,platycodon,Prunella,Sanleng,Curcuma,Angelica,Chuanxiong and so on.3.In the treatment of Hashimoto’s thyroiditis complicated by nodules,Professor Ma Jian put the theory of qi movement up and down and the idea of sparse wood and earth support throughout,and believed that the pathogenesis of this disease is mainly liver stagnation and spleen deficiency,phlegm,qi and blood intertwined. |
