| Objective:1.The uncertainty status of stroke inpatients and their families,in the current context,The possible causes were analyzed by comparing the pre-epidemic data.2.The current status of medication compliance of stroke patients was evaluated to explore the correlation between disease uncertainty and medication compliance of stroke patients.Methods:In order to evaluate the status of disease uncertainty among stroke inpatients and their family members under the background of normalized epidemic prevention and control,selected 191 pairs of patients and their family members who met the inclusion and exclusion criteria from relevant departments of three tertiary hospitals in Wuhan and Huangshi of Hubei Province from October 2020 to April 2021.The patient and family general data questionnaire,mishel uncertainty in illness scale for adults,self-rated depression scale,state anxiety inventory,mishe1 uncertainty in illness scale for family member,social support rating scale and simplified coping style questionnaire were used for survey,two-person data entry,and statistical analysis of data.The results were compared with pre-epidemic knowledge from literature sources.To evaluate the status of medication adherence in stroke patients,to explore the correlation between patients’ disease uncertainty and medication adherence.This part of the study was based on the sampling method of the first part.In addition to the questionnaire survey of the first part,the Chinese version of 8-item Morisky medication adherence scale was added,including face-to-face questionnaire survey,on-site questionnaire recovery,two-person data entry and statistical analysis.SPSS26.0 was used in this study,and case number and percentage were used to describe the general data.Patients with stroke and their family members’ uncertainty in illness and patients’ medication adherence were described by mean and standard deviation.Single factor and stepped-multiple linear regression analysis were used to screen out the influencing factors of disease uncertainty and medication adherence in stroke patients.Pearson correlation analysis was used to explore the correlation between uncertainty in illness and medication adherence in stroke patients,with α=0.05 as the test standard.Results:Part I Research Results:1.The scores of disease uncertainty in stroke patients and their families The uncertainty in illness score of stroke patients ranged from 30 to 95 points,71.35±11.55 points,at a medium level.The score of disease uncertainty of stroke patients’ families ranged from 51 to 112 points,with an average score of82.81±11.37 points,which was at a medium level in general.2.The uncertainty and related situation of patients and their families were compared before and after the epidemic After the epidemic,patients’ sense of disease uncertainty 71.35±11.55,depression 43.86±10.17,anxiety 35.10±7.92,Compared with the pre-epidemic disease uncertainty 74.37±9.22 score,depression 45.76±10.33 score,anxiety50.50±7.01 score decreased(P < 0.05);After the epidemic,the scores of family members’ disease uncertainty 82.81±11.37,depression 33.04±7.79,anxiety34.90±8.28,social support 39.84±7.40,negative coping style 1.00±0.46,Compared with the pre-epidemic disease uncertainty 90.83±9.08,depression39.29±6.10,anxiety 47.55±6.09,social support 41.08±6.43 and negative coping style 1.38±0.56,the results are meaningful(P < 0.05).3.The results of a study on the correlation between stroke patients and their family members’ sense of disease uncertainty Pearson correlation analysis was used to explore the correlation between the disease uncertainty scores of stroke patients and their family members.The results showed that stroke patients’ disease uncertainty was positively correlated with their family members’ disease uncertainty scores(P < 0.05).Part II Research Results:1.Score status of medication compliance in stroke patients The score range of medication adherence of stroke patients was 1.25-8,with an average score of 6.06±1.78,and the middle compliance was the highest,accounting for 42.9%.2.The influencing factors of medication compliance of stroke patients were analyzed Statistical results show that ethnicity,family per capita monthly income,type of medication and disease understanding were the main factors affecting the medication adherence of stroke patients.3.The influencing factors of disease uncertainty in stroke patients were analyzed The results of stepwise multiple linear regression analysis showed that patients’ self-assessment of economic pressure,understanding of the disease,types of current medication,the impact of epidemic prevention and control measures on hospitalization safety,and the impact of epidemic prevention and control measures on disease outcome were the main factors affecting the disease uncertainty of stroke patients.4.Study results of correlation between disease uncertainty and medication compliance in stroke patients The results of Pearson correlation analysis of disease uncertainty and the scores of each dimension and medication adherence in stroke patients showed that there was significant correlation between disease uncertainty and the scores of each dimension and medication adherence(R =-0.168,P<0.05).Conclusions:In this study,it was found that stroke patients and their families still had disease uncertainty,and the disease uncertainty was positively correlated with stroke patients.Comparing the data before and after the epidemic,it was found that the scores of disease uncertainty,depression and anxiety of stroke patients and their families decreased in the current.Under the background of normalized epidemic prevention and control,the medication adherence of stroke patients was moderate.Patients with ethnic minorities,high per capita monthly income,fewer types of current medication,and more knowledge of their own diseases had higher medication adherence.There was a negative correlation between disease uncertainty and medication adherence in stroke patients. |
