Observation On The Clinical Efficacy Of Xiaoyao Powder With Yiguan Decoction In The Treatment Of Type 2 Diabetes Complicated With Chronic Insomnia(Liver Depression And Yin Deficiency Syndrome)

Objective:To investigate the clinical efficacy of Xiaoyao Powder with Yiguan Decoction for treating type 2 diabetes with chronic insomnia(liver depression and yin deficiency syndrome).,observe the patients’ sleep improvement and blood glucose change level,and evaluate the effectiveness,safety and treatment advantages of this method.To provide new ideas and theoretical basis for TCM Intervention in type 2 diabetes mellitus complicated with chronic insomnia.Method:A total of 62 patients with type 2 diabetes mellitus complicated with insomnia from the endocrinology Department of our hospital who met the inclusion criteria were randomly divided into two groups,namely the treatment group and the control group,with 31 patients in each group.The control group was treated with blood glucose control,sleep hygiene education and psychological therapy.On the basis of this,the treatment group took Xiaoyao Powder with Yiguan Decoction one dose per day,oral after breakfast and dinner.Seven days as a course of treatment,a total of 4 weeks of observation.Fasting blood glucose,2h postprandial blood glucose,Pittsburgh sleep quality index(PSQI)score,Insomnia severity index(ISI)score,TCM syndrome score and other related indicators were compared between the two groups before and after treatment,and comprehensive analysis was conducted to draw a conclusion.Results:(1)Xiaoyao Powder with Yiguan Decoction can improve the degree of insomnia patients.After 4 weeks of treatment,the degree of insomnia in both groups was alleviated.The effective rate of insomnia was 83.87% in Xiaoyao Powder with Yiguan Decoction group,67.74% in control group,P<0.05.In terms of the total score of the scale,PSQI and ISI scores in both groups decreased after treatment(P < 0.01),and the total score of each scale in the treatment group decreased more obviously(P < 0.01).In terms of Pittsburgh sleep factor scores,there was no significant difference in sleep disorder factor in the control group,and the scores of other factors were decreased(P < 0.01).The improvement of sleep quality,sleep duration and sleep disturbance in the treatment group was particularly significant(P < 0.01),while there was no significant difference between the two groups of daytime dysfunction after treatment(P > 0.05).(2)Xiaoyao Powder with Yiguan Decoction can optimize the TCM symptoms of patients,effectively reduce the syndrome score.After 4 weeks of treatment,the single symptoms of the two groups were alleviated to different degrees,especially in the main symptoms of upset wakefulness,weakness of waist and knees,hypochondriac pain,in the secondary symptoms of dry mouth and bitter mouth,night sweat and drinking,poor mood and poor appetite,the Xiaoyao Powder with Yiguan Decoction group was better than the control group(P < 0.05).The total score of TCM syndromes in both groups decreased,and the decrease was more obvious in the treatment group(P < 0.01).In terms of TCM syndrome and curative effect,the effective rate of Yiguanjian add and subtractt Decoction group was 80.65%,and 51.61% in control group,P < 0.05.(3)Xiaoyao Powder with Yiguan Decoction can adjust the patient’s blood glucose level.After 4 weeks of treatment,fasting blood glucose and postprandial blood glucose in 2groups were decreased,and the treatment group had more advantages in lowering blood glucose than the control group(P < 0.01).(4)The two groups of patients had no adverse reactions such as rash and gastrointestinal discomfort during the test,and no abnormalities were found in blood and urine routine and physical and chemical indexes such as liver and kidney function.The safety was good.Conclusion:Xiaoyao Powder with Yiguan Decoction has good comprehensive efficacy in the treatment of type 2 diabetes patients with chronic insomnia,which can effectively reduce the degree of insomnia,improve sleep quality,shorten sleep latency,prolong the total sleep time,improve sleep efficiency and improve sleep disorders;Reduce the PSQI and ISI scale score level;Reduce insulin resistance,increase insulin sensitivity,regulate glucose metabolism;Optimize the clinical symptoms and signs of patients,comprehensively regulate the endocrine and metabolic state of the body.To confirm the clinical value of Xiaoyao Powder with Yiguan Decoction in type 2 diabetes complicated with chronic insomnia(syndrome of liver depression and Yin deficiency),it is worth popularizing and applying.
