Clinical Curative Effect Observation Of Linggui Zhugan Decoction And Chaihu Shugan Powder In Regulating The Sub-health Of Liver-qi Stagnation And Spleen-qi Deficiency

Research purposes:This topic is to correct the clinical symptoms of patients with the sub-health of Liver-qi stagnation and Spleen-qi deficiency as the entry point,with Tang Yi,director of Linggui Zhugan Decoction and Chaihu Shugan Powder(later called "Linggu Chaishu Compound Preparation")as the main prescription,to explore the physical distribution characteristics of patients with the sub-health of Liver-qi stagnation and Spleen-qi deficiency as well as the correlation between TCM constitution,syndrome type and curative effect,so as to carry out beneficial exploration for the treatment of clinical sub-health state.Research method:Referring to the relevant literature of Linggui Zhugan Decoction and Chaihu Shugan Powder,the sub-health of Liver-qi stagnation and Spleen-qi deficiency and TCM constitution science.Then the relevant symptom scores and constitution types of patients with the sub-health of Liver-qi stagnation and Spleen-qi deficiency were counted.And the differentiation and treatment experience of Director Tang Yi in treating the sub-health of Liver-qi stagnation and Spleen-qi deficiency was summarized based on the knowledge of studying.The included cases were patients who met the inclusion criteria and received Linggui Chaishu Compound Preparation for treatment of the sub-health of Liver-qi stagnation and Spleen-qi deficiency in the department of preventive treatment of disease in First Teaching Hospital of Tianjin University of Traditional Chinese Medicine from March 2021 to December 2021.To patients with complete informed consent and guide the patients to fill in constitution identification scale,record its syndrome,tongue and pulse condition,statistics,physical classification and TCM symptom integral,patients with oral one 4 weeks,8 weeks after treatment respectively for its reiteration symptoms scale,and 8 weeks after the reiteration of TCM constitution identification of scale,According to the inclusion and exclusion criteria,154 subjects were selected.After statistical analysis and evaluation of the data,the curative effect and the applicable constitution of Linggui Chaishu Compound Preparation were discussed,and the correlation between the curative effect and the basic case data was discussed,and the clinical efficacy of Linggui Chaishu Compound Preparation was observed in the intervention of the symptoms of patients with liver depression and spleen deficiency sub-health state.Excel software was used to establish the case data database,and SPSS25.0 software was used for statistical analysis.Research result:1.A total of 154 sub-health patients with Liver-qi stagnation and Spleen-qi deficiency were included in this clinical observation,including 49 males(31.8%)and 105 females(68.2%).The oldest was 44 years old,and the youngest was 18 years old,with an average age of 36.7±6.067 years old.There were 5 patients aged 18-24 years old(3.2%),58 patients aged25-34 years old(37.7%),and 91 patients aged 35-44 years old(59.1%).BMI ranged from39.96kg/m2 to 15.51kg/m2,with an average BMI of 22.25±2.991kg/m2.There were 114patients(74.0%)in the normal group,27 patients(17.5%)in the ultra recombinant group,7patients(4.5%)in the lean group,and 6 patients(4.0%)in the obese group.2.Among 154 western medicine patients with Liver-qi stagnation and Spleen-qi deficiency,the main diseases were cardiac neurosis(30.52%),chronic gastritis(24.68%)and sleep disorder(20.78%),and neuropathy(9.09%).The main diseases of TCM were palpitation(18.83%),stomach PI(18.18%),insomnia(17.53%),vertigo(10.39%),headache(7.79%)and chest paralysis(5.84%).3.154 sub-health patients with Liver-qi stagnation and Spleen-qi deficiency mainly complained of fatigue(22.73%),dizziness(14.29%),palpitation(9.74%),chest tightness(7.14%),insomnia(6.49%),headache(5.19%)at the first diagnosis.The main pulse symptoms were heavy pulse with little force(54.55%)and thin pulse with little force(25.32%).The main tongue features were red tongue(62.99%)and light red tongue(29.22%).The main body of the tongue was tongue with teeth marks(48.7%)and moderate body(47.4%).The main coating on the tongue was white(42.21%)and thin white(27.92%),followed by white and greasy(15.58%)and yellow and greasy(7.14%).4.The constitution type of the 154 patients with Liver-qi stagnation and Spleen-qi deficiency was biased constitution,and the proportion from high to low was Yang deficiency constitution(29.22%),Qi deficiency constitution(29.22%)and Qi stagnation constitution(17.53%).At the same time,the main type of constitution is compound constitution,and the compound TCM constitution is dominated by Qi deficiency constitution,Yang deficiency constitution and Qi stagnation constitution,and the three are most likely to coexist with each other.There was no difference in the distribution of constitution types in different genders and ages.5.There are 154 patients with the sub-health of Liver-qi stagnation and Spleen-qi deficiency.52 cases(33.77%)were clinically cured,54 cases(35.06%)were effective,35cases(22.73%)were effective and 13 cases(8.44%)were ineffective,the total effective rate was 91.56%.4 weeks,8 weeks after treatment compared with before treatment,change the type of sub-health patients complained of and all have different degrees of significantly reduced symptoms of primary symptom integral,before the treatment,after treatment of 4 weeks and8 weeks after treatment respectively are compared,and two different three groups of integral P less than 0.05,differences were statistically significant.In this study,the most common symptoms(more than 10 cases)were fatigue,sleeplessness,dizziness,palpitation,shortness of breath,chest tightness,headache,abdominal distension,chills,spontaneous sweating and loose stools.The symptoms with significant curative effect(effective rate >90%)were fatigue,sleeplessness,dizziness,palpitation,headache,chills and spontaneous sweating.The symptoms of satisfactory curative effect(80% < effective rate ≤90%)were shortness of breath,chest tightness,abdominal distension,loose stools,rupiousness,poor appetite and stomachache.The symptoms of poor efficacy(effective rate ≤80%)were tinnitus,cough and chest pain.6.There was no correlation between the total efficacy and gender,age and BMI.Moreover,there was no statistical correlation of statistics between the total curative effect and different constitution in patients with the sub-health of Liver-qi stagnation and Spleen-qi deficiency.7.Among the 154 patients with the sub-health of Liver-qi stagnation and Spleen-qi deficiency,the total effective rate of physique transformation was 93.33% in patients with Yang deficiency constitution.The Qi deficiency constitution is 86.67%,and the Qi stagnation constitution is 96.29%,and the effect of Qi stagnation constitution as the best.After 8 weeks treatment,the TCM constitution score of patients with the sub-health of Liver-qi stagnation and Spleen-qi deficiency significantly decrease.The two two comparison before and after 8 weeks treatment,P less than 0.05,the differences were statistically significant.Analysis conclusion:1.The sub-health of Liver-qi stagnation and Spleen-qi deficiency is relatively common in clinical practice,mostly in women,middle-aged and normal BMI patients.Western diseases are mainly cardiac neurosis,chronic gastritis and sleep disorders,while the distribution of traditional Chinese diseases is mainly palpitation,stomach PI disease and insomnia.2.The patients with the sub-health of Liver-qi stagnation and Spleen-qi deficiency treated with Linggui Chaishu Compound Preparation had the pulse symptoms of heavy pulse with little force and thin pulse with little force,and heavy pulse and weak force were the main characteristics of pulse symptoms.In the tongue image,the tongue is mainly red and light red,with teeth marks and white or thin coating on the tongue.3.The main TCM constitutions of the sub-healthy patients with Liver-qi stagnation and Spleen-qi deficiency are Yang deficiency constitution,Qi deficiency constitution and Qi stagnation constitution.The patients are almost all compound TCM constitutions.And the main TCM constitutions of them are also Qi deficiency constitution,Yang deficiency constitution and Qi stagnation constitution.What’s more,the three constitution are easily intermingled.4.Linggui Chaishu Compound Preparation has definite efficacy in correcting the sub-health of Liver-qi stagnation and Spleen-qi deficiency,and has remarkable effect in treating the main complaints and symptoms,with good safety.5.Linggui Chaishu Compound Preparation has positive effect on improving the physique of patients with Yang deficiency constitution,Qi deficiency constitution and Qi stagnation constitution,liver stagnation constitution and spleen deficiency constitution,and has promotion value.6.There are many symptoms and signs of sub-health state.And the TCM treatment is mainly based on syndrome differentiation,with various syndrome,the common types are:phlegm and heat internal disturbance,spleen deficiency and dampness resistance,deficiency of heart and spleen,deficiency of lung and spleen,liver stagnation and spleen deficiency,liver and kidney Yin deficiency,liver Qi stagnation,liver stagnation and fire syndrome.In recent years,with the application of constitution theory in clinic,the clinical observation of sub-health in constitution has been reported frequently.Liver and spleen are the most common pathological sites of sub-health state.It is a new attempt to make a systematic clinical observation on the syndrome of Liver-qi stagnation and Spleen-qi deficiency,one of the common sub-health witness types,and to discuss the efficacy of the prescription of classical combination prescription in the treatment of sub-health.This combination of body and syndrome is one of the important methods in the clinical research of sub-health state and the whole field of preventing disease in the future.
