The Clinical Observation Of Chaihu Shugan Powder Jiawei Fang On Persistent Postural-Perceptual Dizziness Of Liver Depression And Qi Stagnation Type

ObjectiveTo observe the clinical effect of Chaihu Shugan Powder Jiawei Fang on PPPD with liver depression and qi stagnation,and evaluate its improvement on patients’ mood,sleep and TCM syndromes,so as to explore new ideas for clinical treatment of PPPD.MethodsA total of 46 PPPD patients with stagnation of liver-qi were enrolled in this study and randomly divided into treatment group and control group,with 23 cases in each group.Treatment plan: Both groups of patients were given psychological counseling.The control group was treated with escitalopram oxalate tablets,once every morning,with the initial treatment dose of 5mg/ time,and then increased to 10mg/time after 6 days,continuously for 12 weeks.The treatment group was given decoction of Chaihu Shugan Powder Jiawei Fang,once a day,100 ml each time,which was taken warm after meals twice in the morning and evening for 12 weeks.The efficacy evaluation indexes were Dizziness Disability Score(DHI),Hamilton Anxiety Scale(HAMA),Hamilton Depression Scale(HAMD),Pittsburgh Sleep Quality Index(PSQI)and TCM syndrome scores,which were evaluated and recorded before entering the group,at the 4th and12 th week of treatment.All clinical data were entered into Epidata3.1 software,then imported into SPSS 22.0 statistical analysis software,and all data were processed and analyzed to evaluate the curative effect.Results(1)Before treatment,there was no statistically significant difference in gender,age,course of disease,previous medical history,DHI,HAMA,HAMD,PSQI and TCM syndrome scores between the two groups(P > 0.05).After 4 weeks of treatment,DHI score,HAMA score,HAMD score and PSQI score of the two groups decreased compared with those before treatment,especially in the treatment group,and the difference between the two groups after treatment was statistically significant(P <0.05).After 12 weeks of treatment,the DHI score,HAMA score,HAMD score and PSQI score of the two groups decreased further compared with those of the fourth week of treatment,and the scores of the treatment group decreased more significantly.The difference between the two groups after treatment was statistically significant(P<0.05).(2)Comprehensive curative effect: The total effective rate of the treatment group was 91.30%,including 1 case cured,8 cases markedly effective,12 cases effective and 2 cases ineffective.The total effective rate of the control group was 69.57%,among which0 cases were cured,3 cases were markedly effective,13 cases were effective and 7 cases were ineffective.Compared with the control group,the curative effect of the treatment group was more significant,and the difference was statistically significant(P<0.05).(3)TCM syndrome curative effect: After 4 weeks of treatment,the TCM syndrome scores of both groups decreased compared with before,and the difference was statistically significant(P<0.05).After12 weeks of treatment,the TCM syndrome score of the two groups decreased further than that of the fourth week of treatment,and the difference was statistically significant(P<0.05).The total effective rate of the treatment group was 86.96%,among which 0cases were cured,4 cases were markedly effective,16 cases were effective and 3 cases were ineffective.The total effective rate of the control group was 73.91%,among which 0 cases were cured,1 case was markedly effective,16 cases were effective and 6 cases were ineffective.There was no significant difference(P>0.05),and the overall therapeutic effect of the treatment group was better than that of the control group.(4)Safety: One patient in the treatment group had diarrhea,one patient had dizziness,nausea and loss of appetite,and the incidence of adverse reactions was 8.70%.In the control group,3patients developed dizziness,and the incidence of adverse reactions was 13.04%.The incidence of adverse reactions in the treatment group was lower.ConclusionChaihu Shugan Powder Jiawei Fang has a good clinical effect on PPPD with liver depression and qi stagnation,which can significantly improve the clinical symptoms of patients,and the curative effect of the treatment group is obviously better than that of the control group.After treatment,patients’ anxiety,depression and sleep were significantly improved.In the whole observation process,there were no serious adverse events in the Chaihu Shugan Powder Jiawei Fang,which indicated that the Chaihu Shugan Powder Jiawei Fang could be used as a safe and effective treatment for PPPD with liver depression and qi stagnation.
