| Objective: To explore the application value of 3D scanning in breast conserving plastic surgery and implant reconstruction surgery for breast cancer.Part 1: To analyze the scientific feasibility of 3D scanning technology for postoperative evaluation after breast conserving plastic surgery;Part 2: The feasibility of 3D scanning for postoperative evaluation after implant reconstruction surgery was analyzed.Part 3: the volume of 3D measurement and the volume estimated by formula method were compared with Archimedes drainage method to discuss the application value of 3D scanning technology in determining surgical methods before implant reconstruction.Methods: 1.Research Object: This study collected the data of patients who received breast conserving plastic surgery and implant reconstruction surgery for breast cancer in the Breast Disease Diagnosis and Treatment Center of The Affiliated Hospital of Qingdao University from August 2020 to August 2021.The inclusion criteria for the first part were patients with histopathologically confirmed primary breast cancer,no evidence of distant metastasis,breast conserving plastic surgery,and voluntary screening;The exclusion criteria of the first part were patients with severe breast ptosis,patients with lack of clinical data or patients who were lost to follow-up,and patients requiring simultaneous healthy side breast reduction.The inclusion criteria for the second and third parts were histopathologically confirmed primary breast cancer patients,no evidence of distant metastasis,breast cancer implant reconstructive surgery,and voluntary scanning;The second and third exclusion criteria were patients with missing clinical data and lost followup,as well as patients requiring combined autologous tissue flap transplantation or autologous fat transplantation in breast reconstruction surgery.2.Research Methods: The first part: 3D scanner was used to scan the patients’ breasts after breast plastic surgery,and the convexity and volume of the breasts were measured in the image data processing software,and the convexity difference and volume difference of the double breasts were obtained.Breast-Q scale and Harris system were used for postoperative evaluation at the same time after surgery,and statistical analysis was conducted among the evaluation results.The second part: After implant reconstruction,the convexity and volume were also measured in three dimensions,and Breast-Q scale and Harris system were also used for evaluation and statistical analysis.The third part: breast radius and breast height were measured for patients undergoing implant reconstruction surgery,breast volume was calculated by formula method,and preoperative measured volume was obtained by 3D scanning.Drainage method was used to measure the volume of the resected gland.The correlation and difference between the former two and the latter were analyzed statistically.Results: All 165 patients were women.In the first part,the age of patients undergoing breast conserving surgery ranged from 21 to 67 years old,with an average age of43.43±10.15 years;in the second and third parts,the age of patients undergoing implant reconstruction surgery ranged from 29 to 58 years old,with an average age of 43.12±7.50 years.In the study,the time of all preoperative 3D scanning and measurement of breast diameter line was 1 day before or on the day of surgery,and the time of postoperative 3D scanning and scale measurement was 90±10 days.Part 1: the 3D scanning technology is applied to buy milk postoperative evaluation of Breast surgery,result and Breast-Q scale and Harris system were presented significant correlation,whether through calculating milk volume difference or poor crown can be well applied to the symmetry of Breast cancer confirmed Breast surgery and cosmetic effect assessment,and all data statistics have statistical significance;Part 3: 3D scanning technology was applied to evaluate the reconstruction surgery after reconstruction,and the results showed significant correlation with Breast-Q scale and Harris system.It was more reliable to evaluate the effect of reconstruction surgery by calculating the volume difference,and all the data were statistically significant.Part 3: The 3D scanning method was used to measure the volume of the affected side of the breast before implant reconstruction surgery,and the results were highly correlated with the drainage method.Compared with the preoperative formula method,the 3d scanning data were also closer,and the difference was statistically significant.Conclusion: 3D scanning technology has broad application space and practical value in breast conserving surgery and reconstruction surgery.1.The application of 3D scanning technology after breast conserving plastic surgery and reconstruction surgery can provide better follow-up and evaluation,and the evaluation can be quantified,visualized,objective,repeatable and accurate to evaluate postoperative symmetry and other aesthetic effects;2.Preoperative application of 3D scanning technology can serve the clinical practice,participate in surgical design and precise selection of implants,and formulate the most appropriate surgical method by integrating various technologies,including estimation and measurement of breast volume,selection of specific surgical methods and selection of implants,etc.. |
