| Objective: To evaluate the application value of 3d gait analysis system in rehabilitation of patients with lumbar disc herniation. Methods: Thirty-one patients with lumbar disc herniation were selected as the experimental group and 30 healthy volunteers were recruited as the control group.BTS 3d gait analysis system was used to evaluate the gait characteristics of the subjects. The difference of gait between the two groups was analyzed.Spearman correlation coefficient was used to evaluate the correlation between gait parameters and the patients’ digital pain score(NRS),Japanese Orthopedic Association(JOA)score and Oswestry Disability Index(ODI).Results: 1.Compared with lower limb gait in the experimental group, the percentage of lower limb support phase decreased, the percentage of swing phase increased, and the step length became shorter;the range of motion of hip, knee and ankle joints of the affected lower limbs decreased. The first peak value of Ground Reaction Force(GRF)of the affected lower limb decreased.2.Compared with the same side data of the lower limb of the affected side in the experimental group, the percentage of swing phase of the lower limb of the affected side in the experimental group increased and the step length became shorter;decreased range of motion of hip, knee and ankle joints;the first peak value of GRF on sagittal axis and the second peak value on sagittal axis and vertical axis decreased.3.Comparing the data of healthy lower limbs in the experimental group with that in the control group, the percentage of swing phase of healthy lower limbs decreased;the motion of ankle in sagittal plane decreased. The first peak value of GRF decreased on sagittal axis and increased on vertical axis, and the second peak value of GRF decreased on sagittal axis and vertical axis.Meanwhile, compared with the control group, the experimental group’s step speed and frequency decreased and step width increased.4.Correlation analysis showed: Step speed, step length of affected side, range of motion of affected side hip joint on sagittal plane and coronal plane of affected side, range of motion of affected side pelvis on coronal plane, second peak value of affected side GRF on vertical axis and sagittal axis were negatively correlated with VAS score and ODI score, but positively correlated with JOA score. The first peak value of GRF on the vertical axis was negatively correlated with ODI score. The differences were statistically significant. Conclusions: The spatiotemporal parameters, kinematics and dynamics of gait in patients with lumbar disc herniation changed significantly, and some parameters were correlated with the LBP function assessment scale, which could provide a baseline for future research on LDH gait. |
