Objectives1.This study aimed to look at the present state of social support,disease perception,hope,and willingness to return to work in young and middle-aged RRD patients,as well as the factors that influence their readiness to return to work.2.To analyze the relationship between the four variables of social support,illness perception,hope,and readiness to return to work,and to explore the similar mediating effects of illness perception and hope in social support and willingness to return to work,to provide a reference for further development of vocational rehabilitation-related interventions and to provide basic information to promote the early and triumphant return to work of patients.MethodsA total of 422 middle-aged and young RRD patients who were received after surgical treatment in one of two tertiary care hospitals in Tianjin and who were selected as the study population,and questionnaires were used to investigate the general information questionnaire,Social Support Rating Scale,Brief Illness Perception Questionnaire,Herth hope Index and Readiness for Return-To-Work Scale.SPSS 23.0 and Amos 24.0 software were used for data analysis.The statistical methods included: applying descriptive analysis of the current status of general information,social support,illness perception,hope,and readiness to return to work in young and middle-aged RRD patients;applying a nonparametric rank-sum test to analyze the difference of general information in willingness to return to work in young and middle-aged RRD patients;applying Spearman rank correlation analysis The relationship between social support,illness perception,hope and readiness to return to work was analyzed by using Spearman’s rank correlation;multivariate ordered logistic regression was applied to explore the factors influencing the willingness to return to work of middle-aged and young RRD patients;mediating effects analysis was used to study the mediating effects of illness perception and hope in social support and readiness to return to work and to construct a model.Results1.The total score of social support is(35.74±6.88),and the mean score of the entries was(3.57±0.69),which is at a moderately low level.The mean score of each dimension entry was(4.49±0.94)for the subjective support dimension,(3.20±0.80)for the objective support,and(2.73±0.67)for the utilization of support dimension.According to the social support scale level classification criteria,there were 147 cases(34.83%)at the poor level,214 cases(50.71%)at the average level,and 61 cases(14.45%)at the high level.2.The total score of illness perception was(41.62±8.15),and the mean score of the entries was(5.20±1.02),which was at an intermediate level.Among them,the average score of each dimension entry from high to low is the emotional dimension(5.67±1.40),the cognitive dimension(5.16±1.02),and the comprehension dimension(4.48±1.51).3.The total score of the hope level was(32.71±6.39),and the mean score of the entries was(2.73±0.53),which is at a moderately high level.The highest mean score for each dimension entry was maintaining close relationships with others(3.00±0.66),and the lowest score was the dimension of positive attitudes toward reality and the future(2.56±0.65).According to the level of hope according to the ranking criteria,there were 43 cases(10.00%)in the low level,234 patients(55.50%)in the medium group,and 146 cases(34.60%)in the high level.4.The mean scores for the Pre-Intention dimension entries for Readiness to Return to Work were(2.91±0.69),the Intention dimension was(3.44±0.87),the Action Readiness-SelfAssessment dimension was(2.67±1.0),and the Action Readiness-Action dimension was(2.88±0.85).There were 91 cases(21.56%)of pre-intention,205 cases(48.57%)of intention stage,74 cases(17.53%)of action-readiness-self-assessment and 52 cases(12.32%)of actionreadiness-action,which were overall at a low level.5.The results of the univariate analysis showed statistical differences(P<0.05)in age,place of residence,education level,monthly household income per capita,primary carer,mode of employment at work,type of work,occupational long-term eye use,number of hours worked per week,degree of myopia,time of onset,mode of surgery and postoperative complications.6.The results of the correlation analysis showed that social support was positively correlated with readiness to return to work(r=0.494,P<0.001),hope level was positively correlated with readiness to return to work(r=0.449,P<0.001),illness perception was negatively correlated with readiness to return to work(r=-0.483,P<0.001),social support was positively correlated with hope level(r=0.458,P<0.001),social support was inversely correlated with disease perception(r=-0.500,P<0.001)and illness perception were negatively correlated with hope level(r=-0.233,P<0.001).7.The results of the multivariate ordered logistic analysis showed that age(18~29 and30~39 years),education,monthly per capita household income,social support,and hope were protective factors for the readiness to return to work of young and middle-aged RRD patients;time of onset,mode of surgery,complications,type of work,occupational long-term eye use,weekly working hours and perception of illness were risk factors for the readiness to return to work of young and middle-aged RRD patients.8.The structural equation modeling analysis results showed that illness perception and hope partially mediated the relationship between social support and readiness to return to work,with mediating effects of 36.13% and 32.03%,respectively.Conclusions1.Young and middle-aged people with RRD have moderately low levels of social support,moderate levels of illness perception,relatively high levels of hope,and low levels of readiness to return to work.2.Age(18~29 and 30~39 years),education level,monthly household income per capita,time of onset,surgical procedure,postoperative complications,type of work,occupational longterm eye use,and weekly working hours influence readiness to return to work among young and middle-aged RRD patients.3.Social support,illness perception,and hope of young and middle-aged RRD patients are correlated with the readiness to return to work,that is,the higher the patient’s social support,the lower the perception of negative illness,and the higher the level of hope,the better the readiness to return to work.4.Social support directly affects young and middle-aged RRD patients’ readiness to return to work.It can also indirectly influence patients’ willingness to return to work through illness perceptions and hopes.
