Objective: To investigate the polymorphism of SCN5A-H558 R gene of Tibetan patients with atrial fibrillation(AF)in middle and high altitude in Qinghai Province, and analyze the risk factors of AF at different altitudes. Methods: In this study,72 Tibetan patients with AF and 109 Tibetans without AF(NAF)who living at middle altitude in Qinghai were enrolled as the middle altitude AF group and middle altitude NAF group,A total of 102 Tibetan patients with AF and 143 Tibetans without AF who living at high altitude in Qinghai were enrolled as the high altitude AF group and high altitude NAF group,Collecting theirs general clinical data, extracting DNA and identifying genotypes and alleles, using binary logistic regression to determine AF risk factors, then establishing a prediction model of AF by nomogram. Results :1)The SCN5A-H558 R genotype and allele frequencies were statistical differences between the AF and NAF groups(P<0.0125),and the G allele is an independent risk factor for AF(P<0.05),whether at middle or high altitude; There were no statistical differences in the genotype and allele frequencies between the middle and high altitude(P>0.0125);2)Results of the logistic regression analysis,Age,RDW-SD,LAD and G allele were independent risk factors for AF in the middle altitude group. In the high altitude group(P<0.05),Age,Smoking,Hypertension,RDW-SD,FT3,LAD and G allele were independent risk factors for AF in the high altitude group(P<0.05).Conclusions: 1)The occurrence of AF in Tibetan in middle and high altitude of Qinghai is associated with the polymorphism of SCN5A-H558 R.The G allele is an independent risk factor for the occurrence of AF.However, the gene polymorphism has not been obvioused difference between different elevations;2)There are different risk factors of AF between different elevations in Qinghai,low-pressure hypoxia and living habits may play an important role. |
