Correlation Between Triglyceride And Glucose Index And Severity Of Coronary Artery Disease In Patients With Coronary Heart Disease

Objective:In this study,we measured fasting triglycerides and fasting blood glucose levels in patients with coronary heart disease(CHD)and calculated triglyceride glucose index(TyG index),to explore the correlation between TyG index and the severity of coronary artery disease and analyze its predictive value for CHD.Methods: A total of 227 patients who visited the Department of Cardiology,Qinghai Provincial People’s Hospital from November 2019 to November 2021 to be diagnosed with CHD and underwent Coronary Angiography(CAG)examination with complete clinical data were selected as the research subjects,including 109 males and 118 females.According to the results of CAG examination,42 cases with normal CAG or did not meet the diagnostic criteria for CHD were set as the control group.The remaining 185 cases diagnosed with CHD according to the results of CAG were set as the CHD group.Then the severity of coronary artery disease in the CHD group was scored according to the Gensini score.According to the score,the subjects were divided into mild lesion group(0–20 points,n=66),moderate lesion group(21–40,n=63),and severe lesion group(> 40 points,n=56).General data such as age and sex of patients were collected.Laboratory parameters including WBC,Hs-CRP,TC,TG,FBG,Hb A1 c,TB,HDL-C,LDL-C,Lp(a),Scr,AST,ALT,BUN,BUA and Ins.And calculate that TyG index according to the result of FBG and TG.Data processing was performed using SPSS26.0statistical software to compare the differences of TyG index among groups,and Pearson correlation was used to analyze the correlation between TyG index and Gensini score.The risk factors of CHD were analyzed by binary Logistic regression.The ROC curve was used to evaluate the clinical value of TyG index in the prediction of CHD.Results:1.Compared between the control group and the CHD group,there were no significant differences in age,gender,BMI,hypertension history,smoking history,alcohol consumption history,Scr,BUN,BUA,Hb A1 c,TB,AST,ALT,GGT,Lp(a),and Ins(P > 0.05).The differences in diabetes history,WBC,FBG,TC,TG,HDL-C,LDL-C,TyG index and Hs-CRP were statistically significant(P < 0.05),and the TyG index level in the CHD group(9.47±0.76)was significantly higher than that in the control group(8.67±0.55).2.There were no significant differences in age,gender,BMI,history of hypertension,smoking history,BUN,Scr,BUA,Hb A1 c,TB,AST,ALT,GGT,Lp(a),and Ins among the subgroups(P > 0.05),and there were significant differences in diabetes history,drinking history,WBC,FBG,TC,TG,HDL-C,LDL-C,and TyG index(P < 0.05).Compared with the mild lesion group,the levels of WBC and TyG indexes in the moderate lesion group were higher,while the levels of HDL-C and alcohol consumption history were lower.However,the levels of WBC,FBG,TC,TG,LDL-C and TyG indexes in the severe lesion group were higher,and the differences were statistically significant(P < 0.05).Compared with the moderate lesion group,the history of diabetes,FBG,TC,TG,LDL-C,and TyG index levels in the severe lesion group were higher,and the differences were statistically significant(P <0.05).3.There was a positive correlation between TyG index and Gensini score in CHD group(r = 0.467,P < 0.001).4.Binary Logistic regression analysis showed that the increased TyG index was a risk factor for CHD(OR = 2.817,95% CI: 1.522,5.113,P = 0.001).5.The ROC analysis of the TyG index in predicting the occurrence of CHD revealed AUC = 0.792(95% CI: 0.729–0.856,P<0.001).When the TyG index was 9.255,the Jordan index was the largest(0.537),and the corresponding sensitivity and specificity were 63.2% and 90.5%,respectively.Conclusion:1.The TyG index level of patients with CHD is positively correlated with the degree of coronary artery disease.The higher the tyg index level,the more serious the coronary artery stenosis.2.The elevated TyG index is a risk factor for CHD and has certain prediction value for the occurrence of CHD.
