| Research BackgroundMore than 138 million people worldwide have recurrent vulvovaginal candidiasis each year.Exploring the dynamic changes of vaginal microbial species during the onset of recurrent vulvovaginal candidiasis can further clarify the mechanism of RVVC recurrence and hopefully provide a theoretical basis for its clinical treatment.PurposeBy establishing a cohort,we obtained multiple vaginal microbiome samples during the recurrence cycle of RVVC patients,revealed the changes in the microbiome composition of the RVVC attack cycle,and explained the mechanism of RVVC recurrence from the perspective of the changes in the structure and function of the vaginal micro-ecosystem of RVVC patients.Methods14 RVVC patients were recruited,and continuous sampling was required.Finally,6 RVVC patients were included in this research.After analyzing the basic data,the following operations were completed.(1)Establishment of cohort of RVVC patients and analysis of vaginal microbial community structureEstablishing RVVC cohorts with daily collection of vaginal secretions can capture information on dynamic changes in structure and function during intermittentand ictal phases.By BIPES,bacterial 16S rRNA and fungal 18S ITS structural changes in vaginal microbial communities before and after RVVC recurrence were simultaneously analyzed.(2)Draw dynamic bacterial spectrum,screen and determine the key time node of RVVC onsetThe dynamic cohort sampling throughout the year and the dynamic bacterial spectrum map can visually display the flora fluctuation of RVVC patients and provide basic data for determining the key time nodes of RVVC onset.Result1.In the stable stage of patients with recurrent vulvovaginal candidiasis,Lactobacillus is the dominant flora.In the active stage of the disease,the proportion of Lactobacillus is significantly reduced,Gardnerella is the main dominant bacteria,and Prevotella,Bifidobacterium and other genera abundance began to increase.2.When the abundance of Gardnerella began to increase,recurrent vulvovaginal candidiasis began to enter the active phase.Recurrent vulvovaginal candidiasis transitions to a stable phase when the abundance of Gardnerella decreases,and is in a stable phase when it enters the microecological environment with Lactobacillus as the dominant bacteria.3.From the genus level,the composition of the flora of recurrent vulvar candidiasis during the attack is similar to that in the stable phase,with Lactobacillus as the dominant flora.In the 24 days before the attack,the first day There was an increase in Prevotella,and in 6-12 days,Prevotella and Gardnerella were the dominant flora.Conclusion1.In the stable stage of patients with recurrent vulvovaginal candidiasis,Lactobacillus is the dominant flora.In the active stage of the disease,the proportion of Lactobacillus is significantly reduced,Gardnerella is the main dominant bacteria,and Prevotella,Bifidobacterium and other genera abundance began to increase.2.When the abundance of Gardnerella began to increase,recurrent vulvovaginal candidiasis began to enter the active phase.Recurrent vulvovaginal candidiasis transitions to a stable phase when the abundance of Gardnerella decreases,and is in a stable phase when it enters the microecological environment with Lactobacillus as the dominant bacteria.3.The detection of 16SrRNA gene by second-generation high-throughput sequencing can effectively predict the onset time of recurrent vulvovaginal candidiasis and the main inflection point of disease prognosis. |
