| Objective:To explore the influence of oral and mental health on the quality of life of patients with oral lichen planus(OLP)in northeastern Sichuan,in order to provide a theoretical basis for the treatment of OLP patients and the improvement of quality of life.Methods:(1)The clinical data of OLP patients diagnosed in the Department of Stomatology,Affiliated Hospital of North Sichuan Medical College from December 2020 to November 2021 were collected.For OLP patients who met the inclusion criteria,at the initial diagnosis and after 2 weeks of treatment:Visual analogue scale(VAS)and REU scoring system were used to record the oral health status of patients and OLP;Self-Rating Anxiety Scale(SAS)and Self-Rating Depression Scale(SDS)were used to investigate the mental health status of OLP patients;Oral Health Impact Profile(OHIP-14 Chinese version)was used to investigate the quality of life of the patients.The correlation analysis of mental health status,oral health status and quality of life of the patients included in the study was carried out at the initial diagnosis and after 2 weeks of treatment.In addition,compared with the initial diagnosis,it was verified whether the oral and mental health of the patients improved after 2 weeks of treatment,and on this basis,the changes in the quality of life of the patients were observed,that is,the impact of oral and mental health on the quality of life of the patients.(2)29 patients selected from the 48 patients by random number table method were observed the expression percentage of T lymphocytes(CD3~+,CD4~+,CD8~+,CD4~+/CD8~+)by flow cytometry at the initial diagnosis and after 2 weeks of treatment.Results:(1)48 patients were included in the study,9 males accounted for 18.75%,39 females accounted for 81.25%,and the age was mainly concentrated in 30-60 years old.At the initial diagnosis,the anxiety score,depression score,VAS score,and REU score of OLP patients were significantly positively correlated with the total score of OHIP-14 scale,and the correlation coefficients were:0.532,0.488,0.699 and 0.388,respectively.After 2 weeks of treatment,the anxiety score,depression score,VAS score,REU score of OLP patients were significantly positively correlated with the total score of OHIP-14 scale,and the correlation coefficients were:0.504,0.591,0.442 and 0.716,respectively.(2)Compared with the initial diagnosis,after 2 weeks of treatment,the following changes occurred in the examination indicators:①Analysis of the patient’s mental health:SAS and SDS scores were significantly reduced(P<0.001),which proved that the patient’s mental health level after treatment.②OLP-related oral health analysis of patients:VAS value and REU value were significantly decreased(P<0.001),which proved that the oral health level of patients was indeed improved significantly after treatment;③Oral health-related quality of life assessment:Among them,the detection results of peripheral blood T lymphocyte subsets of 29 patients selected by the random number table method showed that compared with the initial diagnosis results,the percentages of CD3~+ and CD4~+ and the ratio of CD4~+/CD8~+ were significantly increased(P<0.001),and the percentage of CD8~+ was decreased.(P<0.01).Changes in patients’ biological markers also demonstrated improvements in oral and mental health after treatment;④OHIP-14 score was significantly decreased(P<0.001).Due to the improvement of oral and mental health after treatment,the patients’ oral health-related quality of life also improved significantly.Conclusion:The occurrence and development of OLP is closely related to anxiety,depression and immune factors.Psychological counseling and conventional treatment can relieve anxiety and depression symptoms,promote lesion healing,reduce oral pain,and improve the mental and oral health of patients,which is beneficial to the quality of life of OLP patients. |
