| Objective: the purpose of this study is to treat patients with melancholy and traumatic chronic tension headache(Chronic tension-type headache,CTTH)by transcranial magnetic stimulation(transcrania lmagnetic stimulation,TMS)combined with Shengyi traditional Chinese medicine,to observe the indexes of heart rate variability(Heart rate variability,HRV)before and after treatment,and to analyze its preventive value in physical and mental diseases.To explore the efficacy of transcranial magnetic stimulation combined with Shengyi traditional Chinese medicine in relieving pain,calming mind,improving memory,attention,association and imagination.Methods: 90 patients with chronic tension headache who met the inclusion criteria of this study were randomly divided into control group(treated with TMS)and experimental group(treated with traditional Chinese medicine on the basis of control group).There were 45 patients in each group for 4 weeks.The clinical efficacy of the two groups before and after treatment was evaluated by headache symptom score scale(Appendix 1),melancholy headache TCM symptom score scale(Appendix 2),injury degree integral scale(Appendix 3)and HRV index(time domain index: SDNN,RMSSD;frequency domain index: LF,HF,LF/HF).At the same time,pay close attention to the possible adverse reactions of patients during the treatment,timely treatment and record.Results: there were 6 cases of shedding during the treatment,44 cases in the treatment group and 40 cases in the control group.There was no significant difference in general data between the two groups(P > 0.05),and it was comparable.1.Headache symptom score scale score: comparison within group 1: the headache symptoms and total scores of the two groups after treatment were significantly lower than those before treatment(P < 0.01).Compared with the control group,the headache frequency and total headache score of the treatment group were significantly lower than those of the control group(P < 0.01),but there was no significant difference between the control group and the control group(P >0.05).3 headache effect: the total effective rate was 97.7% in the treatment group and 77.5% in the control group,with statistical difference(P < 0.01).2.Score of TCM syndrome score scale of melancholy traumatic headache:comparison within group 1: after treatment,the TCM syndrome score of melancholy traumatic headache in the two groups was significantly lower than that before treatment(P < 0.01).The score of TCM syndrome in the treatment group was significantly lower than that in the control group(P < 0.01).3 curative effect of TCM syndrome of sad headache: the total effective rate was 95.4% in the treatment group and 72.5% in the control group,with statistical difference(P <0.01).3.HRV index analysis: time domain index: SDNN value showed an upward trend from 35.07±10.49 to 41.37±10.57.The RMSSD value also showed an upward trend from 32.91±12.52 to 39.81±13.83.The improvement of SDNN in these two time domain indexes was significantly different from that in the control group.It is suggested that the regulation ability of sympathetic nerve,the activity of parasympathetic nerve and the activity of autonomic nerve function are significantly improved.Frequency domain index: LF index increased from 273.45±288.78 to 645.33 ±518.06,HF index increased from 418.32±375.12 to 526.24±460.20,LF/HF index increased from 0.65±0.36 to 1.30±0.27,showing a very significant difference.There was a very significant difference in the improvement of LF/HF between the frequency domain index and the control group(P < 0.01).It is suggested that there is a very significant improvement in LF/HF index,and the overall balance between sympathetic nerve and parasympathetic nerve is significantly improved.Conclusion: the therapeutic effect of transcranial magnetic stimulation combined with Shengyi Powder is better than that of transcranial magnetic stimulation alone.on the one hand,it can effectively improve the heart rate variability and the activity of vagus nerve and sympathetic nerve.thus,it can improve the ability of autonomic nervous system to regulate the heart and protect cardio-cerebrovascular system,and reduce the risk of autonomic nerve dysfunction and cardio-cerebrovascular disease.And the balance of sympathetic and parasympathetic nerves tends to develop in the direction of sympathetic dominance,which can effectively improve the activity and balance of sympathetic and parasympathetic nerves in patients with melancholy and sadness.On the one hand,it proves the advantage of choosing Shengyi Powder according to aspiration syndrome differentiation to treat physical and mental illness,which can reduce pain symptoms,adjust physical and mental state,and get rid of melancholy mood. |
