Clinical Observation Of Modified Chaihu Shugan Powder In The Treatment Of Chronic Tension-type Headache Of Liver Depression And Qi Stagnation Type

ObjectiveTo observe the clinical curative effect of Chaihu Shugan Powder on patients with CTTH（liver depression and qi stagnation type）and evaluate its safety,as well as its improvement of common clinical symptoms（anxiety,depression,insomnia,etc.）and TCM symptoms for such patients,and hope to give full play to the advantages of TCM and enrich the clinical treatment scheme of CTTH patients through TCM syndrome differentiation and treatment.MethodsIn this study,52 patients with CTTH（liver stagnation and qi stagnation）were randomly divided into control group and treatment group,with 26 cases in each group.The control group was treated with citalopram hydrobromide tablets,20mg/time,once a day in the morning,for 8 weeks as a course of treatment.The treatment group was given citalopram hydrobromide tablets orally（20mg/time,once a day in the morning）+modified Chaihu Shugan powder,and a course of treatment lasted for 8 weeks.This prescription is decocted by Chinese pharmacy,one dose per day,twice orally,once in the morning and once in the evening,100 ml each time.Before and after 8 weeks of treatment,the changes of headache（total headache score,headache degree score,average monthly headache score,headache duration score and accompanying symptom score）,emotional state（changes of HAMA and HAMD score）,sleep state（changes of PSQI score）and TCM symptoms（improvement of TCM syndrome score）of the two groups were evaluated,and the safety of the two groups was evaluated,processed and recorded.Results（1）A total of 52 CTTH patients were included in this study,including one shedding case in the treatment group and the control group,and the final effective cases were 25 in both groups.（2）Compare the age,sex,course of disease,headache related scores,HAMA scores,HAMD scores,PSQI scores and TCM syndrome scores between the two groups before treatment.The difference is not statistically significant（P>0.05）and comparable.（3）After 8 weeks of treatment,the headache-related scores,HAMA scores,HAMD scores and PSQI scores in the treatment group and the control group all decreased compared with those before treatment,and the degree of decrease in the treatment group was more obvious,with statistical significance（P<0.05）.（4）Statistic and compare the total clinical effective rate of headache after 8 weeks of treatment between the two groups.The total effective rate of the treatment group is 92%and that of the control group is 76%,that is,the overall effect of the treatment plan in the treatment group is better than that in the control group.（5）After 8 weeks of treatment,the scores of TCM syndromes in both groups decreased compared with those before treatment,and the differences between the two groups were statistically significant（P<0.05）.Comparing the improvement degree of TCM syndromes between the two groups after treatment,the total effective rate of the treatment group is 88%,and that of the control group is 68%.The difference between the two groups is statistically significant（P<0.05）,that is,the TCM syndromes of the two groups have been improved in different degrees,but the clinical total effective rate of the treatment group is higher,and the improvement of TCM symptoms is more obvious.（6）During the study,no drug-related adverse reactions and side effects occurred in the patients of the two groups.ConclusionThis study found that the combination of Chaihu Shugan Powder and western medicine has a good clinical effect on CTTH patients with liver depression and qi stagnation,and it has more advantages than western medicine alone.Chaihu Shugan Powder Combined with Citalopram Hydrobromide Tablet can reduce the degree of headache,reduce the number of monthly headache attacks and shorten the duration of a single headache,and at the same time,it can also improve the mood and sleep of patients with anxiety and depression.This combination of traditional Chinese and western medicine is effective in treating CTTH patients with liver depression and qi stagnation.Chaihu Shugan Powder Jiawei Prescription has no drug-related adverse reactions during the use of this study,which is safe and can be further popularized clinically.More CTTH patients and even other types of headache patients can benefit through flexible addition and subtraction.
