Objective By studying the correlation between plasma arteriosclerosis index(AIP)and coronary heart disease(CHD)complicated with HF,we provide a new index for clinical evaluation of CHD complicated with HF.Methods According to the inclusion and exclusion criteria,a total of 52 patients diagnosed with CHD and 56 patients with CHD combined with HF were continuously included in the cardiology Department of the Second Clinical Medical College of Inner Mongolia University for Nationalities from December 2021 to December 2022.A preliminary experiment was performed to determine whether a AIP was an influencing factor for CHD combined with HF,and then a formal experiment was conducted.A total of 149 patients diagnosed with CHD combined with HF were selected and divided into cardiac function grade I group(n=34 cases),cardiac function grade II group(n=37 cases),cardiac function grade III group(n=37 cases)and cardiac function grade IV group(n=41 cases)according to NYHA cardiac function grade.The general and clinical data of CHD combined with HF were compared.Correlation analysis of clinical conditions of patients with different cardiac function grades;Disordered logistics regression analysis affecting cardiac function classification;a AIP assisted in the evaluation of ROC curve analysis of different grades of cardiac function.All data were analyzed using SPSS22.0 software package,and multi-factor disordered logistic regression was used to screen the risk factors of CHD with HF.α=0.05 was used as the test level,AIP=log[TG(triglyceride)/HDL-C(high-density lipoprotein cholesterol)].To avoid negative AIP,Adjustments were made "x 10" before statistical analysis,and the adjusted a AIP=log[TG(triglyceride)/HDL-C(high-density lipoprotein cholesterol)×10].Results Preliminary experimental results: A total of 52 patients with CHD and 56 patients with CHD combined with HF were collected in this preliminary experiment.logistic regression equations were constructed by BMI,NT-pro BNP,LVEF,a AIP,etc,It was concluded that BMI,smoking,NT-proBNP,UA,LVEF and aAIP were risk factors(P < 0.001).Official experimental results:1.There were no significant differences in age,sex,drinking history,hypertension and diabetes among CHD and HF groups,but there were differences in BMI and smoking history.2.The clinical data of CHD combined with HF group were statistically significant in NT-pro BNP,creatinine,uric acid,LVEF and a AIP.3.Correlation analysis of clinical conditions of patients with different cardiac function grades: NT-pro BNP,uric acid,creatinine,a AIP were positively correlated with NYHA cardiac function grades;It was negatively correlated with LVEF.4.Disordered logistics regression analysis affecting cardiac function classification: BMI,NT-pro BNP,LVEF,a AIP and other factors were included in the construction of multi-factor Logistic regression equation.The results showed that NT-pro BNP,a AIP and smoking were statistically significant in grade II cardiac function compared with grade I cardiac function.In grade III cardiac function,compared with grade I cardiac function,NT-pro BNP,a AIP and smoking were statistically significant.NT-pro BNP,a AIP,smoking,and BMI were statistically significant in grade IV compared with grade I.5.ROC curve analysis of a AIP to assist in the evaluation and identification of different cardiac function grades: The sensitivity and specificity of ROC curve for the diagnosis of HF correlation between Grade I cardiac function group and Grade II cardiac function group were 76% and 68%,and the AUC under the curve was 0.755(95% confidence interval 0.642--0.869),and the diagnostic point was 1.11(P < 0.05).The sensitivity and specificity of ROC curve for diagnosing the HF correlation between Grade III cardiac function group I and CHD were 81.1% and 82.4%,and the AUC under the curve was 0.863(95% confidence interval 0.778 to 0.949),and the diagnostic point was 1.19(P < 0.05).The sensitivity and specificity of ROC curve for diagnosing a AIP with coronary heart disease combined with heart failure were 80.5%,94.1%,0.920(95% confidence interval,0.856 to 0.984),and 1.30(P<0.05)for differentiating Grade IV cardiac function group.Conclusion 1.There is a correlation between aAIP and coronary heart disease combined with heart failure.2.a AIP is an independent risk factor for coronary heart disease combined with heart failure.3.a AIP is valuable in the evaluation of cardiac function grading,which can assist in the identification of different cardiac function grading.
