| Objective1.To investigate the status of stress perception in patients with coronary heart disease after PCI,and to evaluate their stress perception.2.To explore the patient ’s stress perception and psychological status through qualitative interviews,refine and summarize the theme;3.Based on the theory of stress and coping,a stress perception coping plan for patients with coronary heart disease after PCI was constructed.Methods1.Cross-sectional survey : From May to September 2022,patients with coronary heart disease after PCI who met the inclusion criteria in the department of cardiovascular medicine of a top three hospital in Jinan,Shandong Province were selected by convenient sampling method.Research tools include general information table,stress perception scale and Pittsburgh sleep quality scale.The survey data were analyzed by SPSS24.0,including descriptive statistical analysis,one-way ANOVA,and Pearson correlation analysis.2.Qualitative interview : The maximum variation sampling method was used to select patients with coronary heart disease within 3 months after PCI in the outpatient or inpatient department of cardiovascular medicine of a tertiary hospital in Jinan City,Shandong Province.The sample size of qualitative research is until the interview information reaches saturation.In this study,10 patients were selected.After obtaining the informed consent of the patients,the interview was conducted according to the content of the interview outline through face-to-face individual interviews.3.Delphi method : In this study,16 experts were selected for consultation.After obtaining the consent of the experts,the first round of consultation questionnaires were issued,including the basic information of the experts,the self-assessment table of the authority of the experts,the overall summary table of the response plan,the first-level indicator table of the response plan,the second-level indicator table of the response plan and the overall evaluation table of the response plan.After a certain interval,the second round of expert consultation was conducted,and finally the corresponding plan was revised and changed to form the final response plan.Results1.Cross-sectional study : The total score of stress perception in patients with coronary heart disease after PCI was(35.57 ± 6.58),the score of tension dimension was(3.03 ± 0.56),and the score of loss of control dimension was(2.02 ± 0.65).The scores of each dimension of sleep quality : subjective sleep quality(2.20 ± 0.39),sleep time(2.00 ± 0.54),sleep time(1.35 ± 0.53),sleep efficiency(1.35 ± 0.53),sleep disorder(2.51 ± 0.20),hypnotic drugs(2.66 ± 0.56),daytime dysfunction(2.43 ± 0.40).Pearson correlation analysis showed that the two dimensions of stress perception scale,namely,tension,were significantly correlated with the sense of loss of control(P < 0.01).The total score of stress perception in patients with coronary heart disease after PCI was significantly negatively correlated with the dimensions of sleep time,sleep time and sleep efficiency(P < 0.01).The dimension of loss of control was significantly negatively correlated with the subjective sleep quality,sleep time,sleep time,sleep efficiency and daytime dysfunction in the sleep quality scale(P < 0.05).2.Qualitative research : Through Nvivo software analysis,the interview content was summarized into five themes,namely,postoperative rehabilitation training guidance needs,negative emotional relief needs,stress perception response needs,remote follow-up needs,and postoperative health education needs.3.Delphi method : A total of 16 experts were included in the expert correspondence of this study.A total of 16 and 16 questionnaires were distributed in two rounds.The questionnaire recovery rate was 100 % and 100 %.The high recovery rate showed that the experts paid more attention to this study and the experts had higher participation.The authority coefficients of the two rounds of experts were0.89 and 0.91,both greater than 0.8,indicating that the experts had high authority and ensured the reliability of the results.The first round of expert consultation should respond to each other.Conclusion1.The level of stress perception in patients with coronary heart disease after PCI is above the average level,which is closely related to sleep quality and is negatively correlated.2.Through qualitative interviews,the psychological status of patients after PCI was consulted,and four themes were extracted,including the need to relieve stress perception,the need to relieve negative emotions,the need for postoperative rehabilitation training guidance and the need for postoperative health education,in order to provide reference for the response plan.3.Based on the theory of stress and coping,this study extracted the theme through cross-sectional research and qualitative research and preliminarily constructed the coping plan.The coping plan was formulated from the aspects of stress perception coping,negative emotion relief,sleep quality improvement and postoperative rehabilitation guidance,and then revised and supplemented through two rounds of expert consultation,and finally completed,in order to provide reference for clinical workers. |
