ObjectiveTo study the value of some special vascular index on diagnosis of cardiovascular disease,especially on diagnosis of heart function.ContentPART ONE(The evaluation on wave and amplitude of coronary vein by M-mode echocardiography)l.Objective:To study the wave and amplitude of coronary vein with coronary heart disease(CHD) by M-mode echocardiography(ECG).2.Materials and methods: The patients were divided into four groups according to the results of coronary angiography and echocardiography: group 1 were 20 healthy volunteers,group2 were 20 patients with abnormal diastolic function whose systolic function were normal,group3 were consisted of 19 patients with abnormal systolic function,group4 were 21 patients with atrial fibrillation whose systolic function were normal.The diameter(T),amplitude(D) and wave pattem(M) of coronary vein was measured by M-mode echocardiography at apical nonstandard four-chamber view.3.Results:No significant difference was noted among the four groups on the diameter's change of coronary vein.The wave pattern in group 1 and group2 has two peaks. There was only one peak in that of group3 and group4.The amplitude lowerd gradually from group1,group2 to groups.The amplitudealso significantly lowerd in group4.4. Conclusion: The wave pattern and amplitude of coronary vein may reflect the heart function of the patients with CHD.The change on amplitude and wave pattern of the coronary vein was maybe due to the less blood flow of it.The change on patientswith atrial fibrillation was perhaps due to the arrhythmia and higher atrial pressure.PART TWO(Influence of different pulse pressure on cardiovascular system by Doppler Ultrosound)l.Objective:To investigate the influence on cardiovascular system with differentpulse pressure (PP) by Doppler Ultrasound.2.MateriaIs and methods:31 hypertensive patients with PP over 40 mmHg (group A) , 33 hypertensive patients with PP ranged feme 20 mmHg to 40 mmHg (group B) and 25 non-hypertensive patients with PP over 40 mmHg (group C)were enrolled. The thickness of left ventricular (LVT), the diameter of left ventricular (LVD) , the left ventricular myocardial masses (LVM) , the ratio of mitral inflowearly and late velocity in diastole (E/A) , the left ventricular ejection fraction(LVEF) and resistant index of cariod (RI) were detected by Doppler Ultrasound. Coronary angiography were performed in all of the patients. We analysed the difference in the groups and the correlation between PP with LVT(or LVD, LVM, E/A,LVEF.RI) . 3.Results: The PP and RI in group A and group C were obviously higher than that ingroup B (P0.01) . No significant difference was noted between group A and group C. The LVM.LVD and LVT in group A were obviously higher than that in group B or group C (P0.01) . The E/A in group A were obviously lowwer than that in groupB or group C (P0.01) . No significant difference was noted between group B and group C. The LVEF had no significant difference in Group A, B, C;(2) There was well correlation in the PP with the LVM, LVD, E/A, LVT, RI (P0.01) , and the PP had the best correlation with the RI. There was no significantcorrelation in the PP with the LVEF. The incidence rate of coronary heart disease in group C (84%) was higher than that in group A (29%) or group B (9%) .4.Conclusion:High PP by high SBP had more influence on the structure of the heart and high PP by low DBF had higher incidence rate of coronry artery disease. So we should treat the patient differently according to the different condition.PART THREE(Evaluation of shear stress of the common carotid artery by Echo in NIDDM)l.ObjectiverTo investigate the wall shear stress,that is the frictional force acting tangentially to the endothelial surface,in the common carotid artery of the diabetic and control subjects.2. Materials and methods:23 NTDDM subjects(group 1) and 20 control subjects (group 2)were enrolled. None of the participants was hypertensive, hyperlipidemic, or a cigarette smoker.
