Background:Thrombopoietin(TPO)is a major cytokine that regulates the growth of megakaryocytes and induces the generation of platelets.It has been found that TPO is mainly synthesized in the liver and bone marrow microenvironment.TPO concentration is negatively correlated with platelets,and inflammation can also affect the production of TPO.Atherosclerosis with thrombosis are the main etiologies of cardio-cerebrovascular arterial thrombotic diseases,such as acute myocardial infarction and acute cerebral infarction.The ruptures of atherosclerotic plaque can lead to thrombogenesis and inflammation.The concentration of TPO which generate from the liver and bone marrow stromal cells will be rise when the sharp decrease of platelet and massive release of inflammatory factors after thrombosis and inflammation.Object:1.This research is to investigate the correlation between TPO and arterial thrombotic diseases of cardio-cerebrovascular(acute myocardial infarction and acute cerebral infarction),platelet parameters and acute response period protein.2.To explore the correlation between TPO concentration and the severity of acute cerebral infarction,and the different periods of cerebral infarction.Methods:We collected the clinical data,laboratory data and the blood samples to test the TPO concentration.These data and samples are from the patients who admitted to the Seventh Affiliated Hospital of Sun Yat-Sen University from August 2020 to December 2021,including arterial acute thrombotic diseases patients of cardiocerebrovascular,non-acute cerebral infarction patients and health check-up persons.We combined the retrospective case-control study with experimental study to explore the TPO levels in arterial thrombotic diseases patients,and compare the concentration of TPO between acute myocardial infarction and acute cerebral infarction patients.We analyzed the relationship between TPO with the severity of acute cerebral infarction and the different periods of cerebral infarction.Besides,we also used the Correlation analysis to research the correlation between TPO,platelet-related parameters and acute reaction period protein.Results:122 patients are confirmed diagnosis of acute arterial thrombotic diseases of cardio-cerebrovascular,including 41 acute myocardial infarction and 81 acute cerebral infarction,and 35 with non-acute cerebral infarction.1.TPO concentration are elevated in arterial thrombotic disease patients(P<0.05),and there has no significant difference between acute myocardial infarction and acute cerebral infarction(P>0.05).TPO is an independent risk factor for cardio-cerebrovascular arterial thrombotic diseases.TPO has a high predictive effect on the occurrence of thrombotic arterial diseases,the prediction effect of acute cerebral infarction was stronger.2.When arterial thrombotic diseases occurred,Neutrophil-to-lymphocyte ratio(NLR)、Red blood cell distribution width to platelet ratio(RPR)and Platelet distribution width(PDW)will be increased,while Lymphocyte to monocyte ratio(LMR)declined.3.Correlation analysis showed that TPO was positively correlated with C-reactive protein(CRP),Procalcitonin(PCT)and RPR,and negatively correlated with Platelet(PLT)and Mean platelet volume(MPV)(P<0.05).4.The concentration of TPO in moderate to severe acute cerebral infarction patients was higher than mild acute cerebral infarction patients(P<0.05).Acute cerebral infarction patients have higher TPO and PCT levels than those patients with non-acute stage(P<0.05).Conclusion:TPO concentration increased in cardio-cerebrovascular arterial thrombotic diseases,and it is an independent risk factor of cardio-cerebrovascular arterial thrombotic diseases.TPO concentration has no difference between acute myocardial infarction and acute cerebral infarction.TPO is correlated with the severity of acute cerebral infarction,the reduction of PLT and the intensity of inflammation.TPO may be used as a biomarker for cardio-cerebrovascular arterial thrombotic diseases in the future,to provide evidence for clinical intervention in advance and reduce the risk of cardiovascular and cerebrovascular diseases.
