ObjectiveIn this study,we used mindfulness cognitive therapy to intervene in patients with severe acute pancreatitis in intensive care unit(ICU)and observed its effect to provide a basis for clinical health care professionals to develop a psychological intervention program to improve severe acute pancreatitis.MethodsThis study is a quasi experimental study.We selected 88 patients with severe acute pancreatitis(SAP)who were first hospitalized in our hospital from November 2021 to December 2022 and met the admission criteria in a comprehensive tertiary hospital in Qinhuangdao.They were divided into two groups according to the order of admission to the ICU,with 44 patients in the control group and 44 patients in the observation group.Both groups received routine care.The observation group received mindfulness cognitive therapy(We Chat light mindfulness mini program)intervention on this basis,with an intervention period of 4 weeks;Starting from the third day of the patient’s admission to the ICU,the treatment frequency is once per day,and the treatment time is concentrated between 10:00 am to 10:30 am or 16:00 pm to16:30 pm every day.The treatment duration depends on the patient’s condition,and is generally between 15 minutes and 30 minutes.The Demography characteristics of patients were investigated by general data questionnaire;Visual Analogue Scale(VAS),Hamilton Anxiety Scale(HAMA/Hamilton Depression Scale(HAMD),Five Face Mindfulness Questionaire(FFMQ),and Barthel Index(BI)were used to evaluate patients’ pain,anxiety,depression,mindfulness level and activities of daily living ability before,during(2 weeks of intervention),and after(4 weeks of intervention)nursing interventions;Before discharge,the Newcastle Satisfaction with Nursing Scales(NSNS)was used to assess the patient’s nursing satisfaction;And compare the hospitalization time and delirium incidence rate between the two groups of patients.After data collection,SPSS 23.0 statistical software was used to process the data,and mean±standard deviation,frequency,and component ratio were used for statistical description;Statistical analysis was conducted using x2 test,independent sample t-test,and repeated measurement analysis of variance,with P<0.05 as the difference with statistical significance.Results1.During the research process,one patient in the control group who underwent tracheal intubation mechanical assisted ventilation due to respiratory failure was excluded;Two patients in the observation group who were discharged before the intervention period of four weeks were excluded;To ensure the control effect,one patient was randomly excluded from the control group,and the final sample size was 84 cases,with 42 cases in each group.There was no significant difference in clinical baseline data between the control group and the observation group,including age,gender,educational level,income level,medical insurance category,incidence factors,and APACHE-II score(P>0.05);There was no significant difference in pain level(VAS score),anxiety level(HAMA score),depression level(HAMD score),mindfulness cognition level(FFMQ score),and ADL(Barthel index score)between the two groups of patients before intervention(P>0.05).2.After the intervention,the VAS score,HAMA score and HAMD score decreased and the FFMQ score and Barthel Index scores increased in both groups,and with the extension of the intervention time,the VAS score,HAMA score and HAMD score gradually decreased and the FFMQ score and Barthel Index scores gradually increased,the probability of delirium and ICU hospitalization time as well as the total hospitalization time of the patients in the observation group were lower than that of the control group,and satisfaction was higher than that of the control group,all differences were significant and statistically significant(P<0.05)ConclusionsMindfulness cognitive therapy is a safe and effective intervention measure suitable for patients with severe acute pancreatitis.Compared with routine nursing,it can effectively improve the negative emotions and pain levels of patients with severe acute pancreatitis,and improve their mindfulness cognitive level and daily living activities;It can effectively reduce the incidence of delirium in patients with severe acute pancreatitis,shorten ICU hospitalization time and total hospitalization time,improve patient prognosis,and increase satisfaction.It is worth further in-depth clinical research.
