| ObjectivesTo know the incidence of hypothermia during anesthesia recovery in patients undergoing laparoscopic surgery;The dynamic changes of body temperature,circulation indexes and anesthesia recovery time of laparoscopic surgery patients during the awakening period of anesthesia were analyzed by different temperature ranges of the inflatable warming system;To clarify the effects of different temperature ranges of inflatable warming system on body temperature,circulation indexes and awakening time of patients undergoing laparoscopic surgery during anesthesia awakening;To explore the appropriate temperature to ensure that patients with laparoscopic surgery during the awakening period of anesthesia maintain normal body temperature,circulation indexes and anesthesia awakening time.MethodsFrom December 2021 to October 2022,90 laparoscopic surgery patients who met the criteria for laparoscopic surgery in the anesthesia resuscitation room of a tertiary A provincial hospital in Dalian City,Liaoning Province were selected as research subjects.The temperature of the operating room was set at 24℃,and the humidity was 40%~60%.Intraoperative blood transfusion was performed using a liquid heater,and the temperature was set at 37℃.The rinsing solution was heated to 38℃ using a thermostat.The investigator numbered the patients according to the order in which the patients entered the anesthesia resuscitation room,used the computer to generate 90 random numbers,corresponding to the patient’s number one by one,and then arranged the random numbers from small to large,and the patients corresponding to the serial number 1~90 were evenly divided into three groups: high-end group(43℃),mid-range group(40℃)and low-grade group(37℃).Research tools include General Information Questionnaire,Steward Awakening Scale,Thermal Comfort Scale,Inflatable Warming System,ECG Monitor,Ventilator,and Ear Temperature Gun.After the patient enters the anesthesia resuscitation room,connect the ventilator,ECG monitor,and inflatable warming system,monitor heart rate,blood pressure,blood oxygen saturation,immediately cover the heating blanket,observe and record the patient’s body temperature,heart rate,blood pressure,blood oxygen saturation and chills level every 10 minutes from the time of entering the anesthesia resuscitation room and every 10 minutes after entering the anesthesia resuscitation room until 60 min,wait for the patient to be awake,the vital signs are stable and the Steward awakening score is ≥ 4 points,it is judged that the patient can remove the warming blanket when leaving the anesthesia resuscitation room,and record the anesthesia recovery time,extubation time,thermal comfort score.SPSS25.0 statistical software was used for descriptive statistical analysis,one-way ANOVA,repeated measure ANOVA,chi-square test,Kruskal-Wallis test,and the difference was statistically significant with P<0.05.Results1.The incidence of hypothermia during anesthesia recovery in the three groups was 56.7%,all of which were mildly hypothermia.Among them,the incidence of hypothermia was 66.7% in the high-end group,53.3% in the middle-range group,and50% in the low-grade group.The incidence of chills during anesthesia awakening in the three groups was 42.2%,among which the incidence of chills in the high-end group was53.3%,the incidence of chills in the middle-range group was 40%,and the incidence of chills in the low-grade group was 33.3%.There were no significant differences in the incidence of hypothermia and chills during anesthesia awakening between the three groups(P>0.05).Through the comparison of chilling levels of chilling patients,the patients with chills in the high-grade group were relieved when they were admitted to the anesthesia resuscitation room for 40 minutes,and some patients in the middle and lowgrade groups had not been relieved when they entered the anesthesia resuscitation room for 60 minutes.2.The difference in preoperative basal body temperature between the three groups was not statistically significant(P>0.05),which was comparable.The body temperature of patients in the high-grade group was(35.76±0.31)℃ when entering the anesthesia resuscitation room,and the body temperature(36.92±0.20)℃ when entering the anesthesia resuscitation room for 60 minutes;The body temperature of patients in the middle range group was(35.81±0.40)℃ when they entered the anesthesia resuscitation room,and the body temperature(36.40±0.37)℃ when they entered the anesthesia resuscitation room for 60 minutes.The body temperature of patients in the low-grade group was(35.87±0.43)℃ when they entered the anesthesia resuscitation room,and the body temperature(36.13±0.41)℃ when they entered the anesthesia resuscitation room for 60 minutes.The temperature of all three groups eventually increased.When the patients were admitted to the anesthesia resuscitation room for 20 minutes,the temperature difference between the high-grade group and the low-grade group was statistically significant(P<0.05),and the temperature difference between the three groups was statistically significant(P<0.05)when the patients were admitted to the anesthesia resuscitation room for 50 minutes.3.When the three groups of patients were admitted to the anesthesia resuscitation room for 20 min,the temperature increase value of the high-end group patients was(0.42±0.06)℃,the temperature increase of the middle-range group patients was(0.20±0.00)℃,and the temperature of the low-grade group patients was(0.11±0.04)℃.When the three groups of patients were admitted to the anesthesia resuscitation room for40 minutes,the body temperature increase value of the high-end group patients was(0.40±0.11)℃,the temperature increase of the middle-range group patients was(0.20±0.00)℃,and the temperature of the low-grade group patients was(0.09±0.04)℃.When the three groups of patients were admitted to the anesthesia resuscitation room for60 minutes,the temperature increase value of the high-end group patients was(0.34±0.12)℃,the temperature increase of the middle-range group was(0.19±0.05)℃,and the temperature of the low-grade group was(0.06±0.06)℃.There was a statistically significant difference in the increase in body temperature per 20 minutes between the three groups(P<0.05).4.The heart rate and blood pressure of the three groups were different at different time points during the awakening period of anesthesia,and the difference in heart rate and blood pressure between the three groups was not statistically significant(P>0.05).5.The anesthesia awakening time of the three groups was(69.03±4.94)min in the high-end group,(72.50±7.18)min in the mid-range group,and(75.73±6.60)min in the low-grade group,and the difference in anesthesia awakening time between the high-end group and the low-grade group was statistically significant(P<0.05),the extubation time of the three groups was(11.4±3.65)min in the high-end group and(17.2±6.12)min in the middle-range group.The extubation time of the low-grade group was(20.47±6.14)min,and the extubation time of the high-grade group and the middle-grade group and the low-grade group was statistically significant between the three groups(P<0.05).There was no significant difference in anesthesia wake-up time and extubation time between the three groups(P>0.05).6.The thermal comfort scores of the three groups during the awakening period of anesthesia were statistically significant in the high-end group(5.97±1.16),the middlerange group(7.13±1.04)and the low-grade group(4.10±0.80)(P<0.01).Conclusions1.The incidence of hypothermia and chills in laparoscopic surgery patients during the awakening period of anesthesia is high,and the temperature range of the high-end group can relieve the chills of patients in the shortest possible time,and medical staff should pay attention to the temperature protection of laparoscopic surgery patients.2.The influence of the three groups of temperature levels of the inflatable heating system on the body temperature and circulation indexes of patients undergoing laparoscopic surgery during the awakening period of anesthesia made the body temperature of the three groups of patients show an upward trend,among which the body temperature of patients in the high-end group rose the fastest,followed by the middlegrade group,and the slowest in the low-grade group.The three sets of temperature levels can effectively maintain the stability of the patient’s heart rate and blood pressure during the awakening period of anesthesia,ensure the stability of the patient’s vital signs,and enable the patient to pass the entire anesthesia awakening period safely and smoothly.3.The effects of the three groups of temperature ranges of the inflatable heating system on the anesthesia awakening time and extubation time of patients undergoing laparoscopic surgery during the anesthesia recovery period were that the anesthesia awakening time and extubation time were the shortest in the high-end group and the relatively long time in the mid-range group and low-grade group.4.The three temperature levels of the inflatable heating system can effectively improve the comfort of patients undergoing laparoscopic surgery during the awakening period of anesthesia,with the middle range group having the highest comfort,followed by the high-end group,and the relatively poor low-grade group.5.The study of three groups of temperature ranges of inflatable heating system on patients undergoing laparoscopic surgery during the awakening period of anesthesia showed that the high-grade group was suitable for patients with general anesthesia without extubation,body temperature below 36.0℃ and chills when patients entered the anesthesia resuscitation room.The mid-range group is suitable for patients who have been extubated under general anesthesia when they enter the anesthesia resuscitation room,and their body temperature is lower than 36.0℃ without chills;The low-grade group is suitable for patients who have been extubated under general anesthesia and have a body temperature above 36.0℃ and no chills when they enter the anesthesia resuscitation room;It is the most suitable temperature choice for stable body temperature and circulation indexes of patients undergoing laparoscopic surgery during the awakening period of anesthesia. |
