PurposeBased on the Traditional Chinese Medicine Inheritance Computer System(TCMICS)V3.0,to explore the acupoint selection rule of acupuncture in the treatment of upper limb motor dysfunction after stroke.MethodsAll clinical trials from the CNKI,WANFANG MED ONLINE,Sino Med,Pub Med and Cochrane databases were searched by computer from establishment to April 2022.The retrieved Chinese and English offerings were respectively imported into Endnote X9 literature management software and screened strictly according to inclusion and exclusion criteria.The basic information of eligible literatures was imported into WPS2022 Excel table to establish the prescription database of acupuncture for upper limb motor dysfunction after stroke after screening.Finally,author information and acupuncture prescription were modified and imported according to the input requirements of TCMICS V3.0,and relevant data were analyzed and integrated through the functions of "acupoint frequency","acupoint combination" and "acupoint clustering" under the module of "acupoint analysis" of the platform.Results1.Literature inclusionA total of 2457 articles were obtained from Chinese and English databases,including 2238 in Chinese and 219 in English.After strict screening,a total of 124 Chinese and English literatures meeting the standards were obtained.There are 116 articles in Chinese database and 8 articles in English database.2.Descriptive analysis of acupoint frequencyA total of 124 acupuncture prescriptions were obtained,including 122 major acupoints commonly used,with a total frequency of 874 acupoints.According to the analysis of the frequency of use of the main acupoints,the top ten acupoints were in turn Jianyu,Hegu,Quchi,Waiguan,Neiguan,Hand Sanli,Binao,Jianzhen,Jianliao and Sanyinjiao.In the analysis of channel normalization,the top five meridians used frequently were the large intestine meridian of hand Yangming,Sanjiao meridian of hand Shaoyang,small intestine meridian of hand Taiyang,Du Meridian and Tou point line.In the analysis of specific acupoints,a total of 76 acupoints belonged to specific acupoints,accounting for 62.30% of the total number of acupoints.And the top five specific points used frequently are intersection point,five transfusion point,eight pulse intersection point,Luo point and original point.According to the analysis of the distribution of acupoints on the human body,the top three parts were the upper limbs,shoulders and heads.3.Analysis results of acupoint combinationWhen the number of support degree is set to 33 and the support degree is 26%,the result is reasonable.6 prescriptions and 12 association rules concerning 4 acupoints involving "Jianyu","Hegu","Quchi" and "Waiguan" were obtained.The association rules are derived by the function of "acupoint export" of the application platform.When Quchi and Hegu appeared in the prescription,the occurrence probability of Jianyu was87%.4.Results of acupoint cluster analysisIn the function of "acupoint clustering",the number of clustering was set as "3",that is,124 prescriptions were divided into 3 categories(3 core acupoint groups),and the results were more reasonable.Conclusion1.Through the descriptive analysis of the frequency of acupoints in this study,it is found that the acupoints with the highest frequency are Jianyu,Hegu and Quchi points;the meridians with the highest frequency are the large intestine meridian of hand Yangming;the specific acupoints with the highest frequency are the intersection points;the selected acupoints are mainly local acupoints of the upper limbs.2.In this study,three new prescriptions were finally obtained through the analysis of association rules and clustering of acupoints.They are respectively "Jianyu,Hegu,Quchi,Waiguan,Neiguan" represented by local acupoint selection,"Hand Sanli,Zhongwan,Waiguan,Xiaguan,Shangqu" characterized by abdominal acupoint selection,and "anterior parietemporal oblique line,parietal parietal 2 line,posterior parietal temporal oblique line,Jianzhen,and Binao" characterized by head acupoint selection.In these three new prescriptions,local acupoint selection had the strongest correlation degree and the highest confidence.3.According to the research results of this subject,although local acupoints are still used to treat upper extremity motor dysfunction of stroke in clinical practice,with the continuous deepening of theories such as "emitting Qi on Shenque point" and "Intersection of collaterals linking sinews",abdominal acupuncture and head acupuncture treatment have become a clinical feature,and they still need to match with local acupoints.In the future,more clinical studies are needed to confirm the effectiveness of head acupuncture and abdominal acupuncture,so as to better serve the clinic. |
