Effect Of Adding And Subtracting Bazhen Decoction On Occult Blood Loss After PLIF For Lumbar Spondylolisthesis

Objective: To analyze the effect of adding and subtracting Bazhen Decoction on occult blood loss after posterior lumbar interbody fusion(PLIF)for lumbar spondylolisthesis through clinical study.To explore the role played by adding and subtracting Bazhen Decoction in the treatment of occult blood loss after PLIF,and to provide a theoretical basis for the clinical use of adding and subtracting Bazhen Decoction in the treatment of occult blood loss after PLIF.Methods: A total of 60 patients with single-segment lumbar spondylolisthesis who underwent PLIF in the spinal surgery ward of Ruikang Hospital,Guangxi University of Traditional Chinese Medicine,between December 2021 and December 2022 were selected,and the patients were divided into a treatment group(30 cases)and a control group(30 cases)using a prospective randomized control method.The control group was treated postoperatively with conventional treatments such as infection prevention,dehydration and swelling reduction,nerve nutrition,and gastric protection,while the treatment group was treated with a 7-day course of adding and subtracting Bazhen Decoction starting 12 hours after surgery on the basis of the control group.The basic information of patients in both groups were measured and recorded before surgery,including name,gender,slipped segment,age,height and weight,and BMI was calculated.hemoglobin(HB)and erythrocyte pressure product(HCT)were checked and recorded before surgery,on the day of surgery,on the third day after surgery and on the seventh day after surgery,and intraoperative blood loss,blood transfusion and postoperative drainage were recorded,and the amount of dominant blood loss and occult blood loss were calculated.The TCM evidence points before and after treatment were counted for both groups of patients.The collected complete test data information was entered and analyzed using SPSS26.0 statistical software,and the calibration standard a=0.05,P<0.05 indicates that the difference is statistically significant.Results: 1.general data comparison: the two groups of patients were compared for gender,slipped segment,age,height,weight,BMI,etc,all P > 0.05,the difference is not statistically significant.2.Comparison of hemoglobin and erythrocyte pressure volume: HB and HCT of the two groups were compared before surgery,on the day of surgery and on the third day after surgery,and the differences were not statistically significant(P>0.05),and HB and HCT of the two groups were compared on the seventh day after surgery,and the differences were all counted significant(P<0.05),and HB and HCT of the treatment group were higher than those of the control group,indicating that adding and subtracting Bazhen Decoction could improve postoperative anemia in PLIF.3.Comparison of explicit blood loss and implicit blood loss: Statistical comparison analysis of the explicit blood loss in the two groups was performed,and the difference was not statistically significant as P > 0.05.Statistical comparative analysis of the amount of occult blood loss on the postoperative day and postoperative day 3 in the two groups was performed,both P>0.05,and the difference was not statistically significant.Statistical comparative analysis of the amount of occult blood loss on postoperative day 7 in the two groups was performed,P<0.05,and the difference was statistically significant,and the treatment group was lower than the control group,which proved that the adding and subtracting Bazhen Decoction could reduce the amount of occult blood loss after PLIF in patients with lumbar spondylolisthesis.4.Comparison of TCM syndrome scores: When the TCM syndrome scores of patients in the two groups were compared before treatment,the difference was not statistically significant(P > 0.05),and when the TCM syndrome scores of patients in the two groups were compared after treatment,the difference was statistically significant(P < 0.05),and the TCM syndrome scores of the treatment group were lower than those of the control group,indicating that the adding and subtracting Bazhen Decoction could improve the symptoms of qi and blood deficiency in patients after PLIF.Conclusion: Adding and subtracting Bazhen Decoction can improve postoperative anemia after PLIF,reduce the amount of occult blood loss after PLIF,and can improve the symptoms of qi and blood deficiency in patients after PLIF with satisfactory efficacy.
