Curative Effect Of Guizhi Wuling Decoction On Cerebral Salt Wasting Syndrome

Objective:The effects of Guizhi Wuling Decoction on cerebral salt wasting syndrome were investigated by monitoring the changes of blood sodium,urine sodium and index recovery time after the treatment of Guizhi Wuling Decoction combined with salt replenishing and other treatments.Method:Forty-two patients who met the diagnostic criteria for cerebral salt consumption syndrome admitted to Wuzhou Hospital of Traditional Chinese Medicine from January 2017 to March 2022 were selected as the study objects.The patients in this group were randomly assigned into control group(21 cases)and treatment group(21 cases)by the envelope method.The control group was treated with sodium supplement and hydration according to the condition of blood sodium,the course of treatment was 10 d.Observation group was treated with Guizhi Wuling Decoction on the basis of control group,the course of treatment was 10 d. The levels of blood sodium,urinary sodium,C-reactive protein(CRP)and brain natriuretic peptide(BNP)were closely monitored during treatment,and the changes of blood sodium,urinary sodium,CRP and BNP were compared between the two groups before and after treatment.The recovery time of hyponatremia and hypernatriuretic sodium in two groups was counted to evaluate the therapeutic effect of hyponatremia,neurological impairment and life ability change before and after treatment.Observe the changes of clinical symptoms and signs of patients with mental malaise,poor tolerance and limb fatigue,and the complications.Statistical analysis was performed using IBM SPSS 23.0 and two-sided test.Results:1 From January 2017 to March 2022,a total of 296 patients with natremia were collected from Wuzhou Traditional Chinese Medicine Hospital,including 42 patients with CSWS.The primary diseases of CSWS patients are mainly spontaneous subarachnoid hemorrhage,brain injury,stroke,intracranial infection,etc.2 After treatment,BUN and BUN/Cr in the treatment group were lower than those in the control group,and the difference was statistically significant(P<0.05).After treatment,the blood sodium and plasma osmotic pressure in the treatment group were higher than those in the control group,and the urine sodium was lower than those in the control group.The 24 h urine volume,24 h urine sodium and 24 h urine chlorine were lower than those in the control group,with a statistically significant difference(P<0.05).There was no significant difference between the two groups in blood potassium and 24 h urine potassium after treatment(P>0.05).3 After treatment,the levels of CRP and BNP in the treatment group were significantly lower than those in the control group(P<0.05).4 The treatment effect of hyponatremia in the treatment group was better than that in the control group,and the recovery time of CSWS was shorter than that in the control group,the difference was statistically significant(P<0.05).5 After treatment,the score of NIHSS in the treatment group was lower than that in the control group,and the score of Barthel index(ADL)in the quality of daily life was higher than that in the control group,with a statistically significant difference(P<0.05).(6)After treatment,the score of symptoms of depression,anorexia and limb weakness in the treatment group was lower than that in the control group,with a statistically significant difference(P<0.05).Conclusion:It is difficult to treat CSWS clinically.Guizhi Wuling Decoction has good curative effect in the treatment of CSWS,which can significantly shorten the course of the disease and improve the state of hyponatrium and hypernatrium,and has a good therapeutic prospect.By analyzing the mechanism of TCM treatment,the author thinks that Guizhi Wuling Decoction has the functions of warming the sun,harmonizing the camp and health,improving water and dampness,which is just in line with the pathogenesis of CSWS.
