Objective:To investigate the occurrence and response status of workplace violence in a general hospital,analyze the influencing factors,changing trends and the reason,evaluate the effectiveness of prevention and control measures of workplace violence in the hospital and put forward suggestions for improvement.Method:The National Case Study Tool on Workplace Violence in the Health SectorQuestionnaire was used as a survey tool to investigate the incidence of workplace violence in the hospital in the past year,and data were collected from surveys conducted in the same hospital in 2018,2019 and 2021 using the same method.Chi-square test and Logistic regression were used to analyze the influencing factors of workplace violence,and chi-square test was used to analyze the changing trend of workplace violence in the hospital from 2018 to 2022.In addition,face-to-face semi-structured interviews were used to analyze the effectiveness of workplace violence measures and subsequent response suggestions.Results:A total of 1346 valid questionnaires were collected in this round of questionnaire survey,with an effective response rate of 92.69%.The total incidence of workplace violence in the hospital decreased from 48.57%in 2018 to 23.33%in 2022(P<0.01),with a decrease rate of 51.97%.The incidence of physical violence decreased from 8.79%in 2018 to 1.56%in 2022(P<0.01),with a decrease rate of 82.25%.The incidence of psychological violence decreased from 47.24%in 2018 to 23.11%in 2022(P<0.01),with a decrease rate of 51.08%.The influencing factors of physical violence include gender,department and whether they are encouraged to report violence.The common responses were "language suppression","informing supervisor" and "confessing to colleagues".The satisfaction rate of incident handling was only 14.28%.The main reasons for not reporting the incident were "thinking it useless","not knowing who to report to" and "afraid of possible consequences".The influencing factors of psychological violence include age,department,contact with patients,employment status and whether they know the violence reporting process,the common response is"language stop","talk to colleagues" and "do not respond",the satisfaction rate of the event processing is only 23.08%,the main reasons for not reporting are "think useless","fear of possible consequences" and "think unimportant"."Safety measures","environment improvement" and "access control" were the most perceived measures taken by hospitals,while "safety measures","training" and "environment improvement"were the most effective measures evaluated.From 2018 to 2022,changes in the demographic characteristics of the subjects were not conducive to evaluating the effectiveness of control measures.The incidence rates of workplace violence were calculated according to demographic characteristics,and all rates showed a downward trend.In addition to safety measures,the hospital has strengthened 11 measures to prevent and control violence,including "environmental improvement","public access control","patient screening","patient protocol","strict restrictions on cash activities","increase in the number of workers","work flow clock system","special clothing and equipment","frequently updated schedule","reduce independent work time" and"training".Contributing to the decline in violence rates,which have contributed to the decrease in the incidence of violence.In addition,the reasons for the decline of workplace violence are also related to the legal support at the social level,the construction of the rule of law,the positive publicity of the media and the improvement of the social security system.Both quantitative and qualitative results suggest that hospitals need to strengthen primary prevention,secondary and tertiary interventions to prevent and control workplace violence.Conclusion:The incidence of workplace violence in the pilot hospital has decreased year by year,and the prevention and control measures taken by the hospital and the society are effective,but the hospital’s handling of violence incidents still needs to be improved,and more perfect systematic prevention and control measures should be continued.
