Objective: The data mining technology was applied to analyze the outpatient medical case of Professor Chen Qiao from the Department of Spleen and Stomach Diseases of Liuzhou Hospital of Traditional Chinese Medicine for the treatment of gastric mucosa and intestinal metaplasia,in order to summarize and inherit the academic thoughts and clinical experience of the tutor,and provide reference for the treatment of gastric mucosa and intestinal metaplasia by traditional Chinese medicine.Methods: 1.Data collection: Outpatient medical records of patients with gastric mucosal intestinal metaplasia diagnosed and treated by Professor Chen Qiao in Liuzhou Hospital of Traditional Chinese Medicine from January 2021 to December 2022 meeting the inclusion criteria were collected,including the patient’s name,gender,age,information of four diagnoses,gastroscopy results,pathological results,diagnosis,constitution typing,TCM syndrome type and prescription.2.Data processing: In order to facilitate the induction and analysis of medical case data and ensure the scientific and credibility of statistical results,the names and symptom descriptions of traditional Chinese medicines in outpatient medical cases were uniformly and standardized,and the standardized data was input into WPS form software(version11.1.0.11194).3.Data Analysis: Upload relevant data to the TCM inheritance computing platform(version V3.0),use data mining technologies such as frequency statistics,rule analysis and cluster analysis to make statistical analysis of the general situation,four diagnosis information,constitution and syndrome,and the rule of medication dispensing prescription in outpatient medical records,and display them in the form of charts.To explore the academic thought and clinical experience of the supervisor in the treatment of gastric mucosa intestinal metaplasia.Results: 1.General information: A total of 128 outpatient medical cases were included in this study,among which 64 were male patients and 64 were female patients,accounting for 50% each.Patients ranged in age from 21 to79 years old,with the highest incidence of 48 cases in the 51-60 age group,accounting for 37.5% of the total.2.Information of four diagnoses: There were 46 clinical symptoms involved in this study,Among them,the high frequency symptoms were abdominal distension(45 cases),acid reflux(41cases),lack of food and lethargy(38 cases),lethargy and fatigue(34 cases),belching(27 cases),dull pain in abdominal region(25 cases),dry mouth and bitter mouth(25 cases),loose stools(24 cases),poor sleep(23 cases),nausea and vomiting(22 cases),pain in the hypochondriac region(20 cases).The tongue image is mainly light tongue and white moss,and the pulse image is mostly thin and weak pulse.3.Constitution and syndrome: The constitution types of GIM patients can be divided into Qi-deficiency(46.09%),Qi-stagnation(23.44%),Yang-deficiency(19.53%),blood-stasis(9.38%)and damp-heat(1.56%).There were 8 types of syndrome,which were spleen and stomach qi deficiency syndrome(34.38%),spleen and stomach deficiency cold syndrome(19.53%),liver and stomach qi stagnation syndrome(15.63%),spleen deficiency and dampness obstruction syndrome(11.72%),liver and stomach heat stagnation syndrome(7.81%),stomach and collaged-blood stasis syndrome(5.47%),deficiency of stomach and Yin syndrome(3.91%)and spleen and stomach dampness-heat syndrome(1.56%).4.Drug use law:The 128 prescriptions that met the standard included a total of 55 traditional Chinese medicines,among which the top ten drugs used most frequently were poria cocos,tangerine peel,sand kernel,glycyrrhiza,atractylosum,pinellia pinellia,ginseng,Radix xylosa,Radix paeony and Radix glycyrrhiza.The drugs with the most frequent use were deficiency supplementing drugs and qi regulating drugs.The drug properties were mainly warm,flat and cold,and the drug tastes were bitter,xin and Gan ju,mostly belonging to spleen,lung and stomach channels.5.Rules of compatibility prescriptions: Based on rule analysis technology,33 commonly used drug combinations such as poring-Tangerine peel,Poring-Glycyrrhiza,Sharen-glycyrrhiza,Sharen-glycyrrhiza,and poring-Sharen-glycyrrhiza were obtained statistically.According to cluster analysis technology,five core prescriptions were obtained: white Rhizoma ractyloides-Radix tangerine-Licorice pinellia-Poria cocos-ginseng,sandalwood-salvia miltiorrhiza-Fructus aurantii-Radix bupleurum-Radix bupleurum-Radix puerariae-Radix puerarii-Radix puerarii-Radix puerarii-Radix puerariae-Radix puerarii-Radix puerariae-Radix puerarii-Radix ophiopogon-Fructus fructus fructus-Radix lycii-Radix chinaberry seed.Conclusions: 1.Professor Chen Qiao believes that the disease of GIM is located in the stomach,which is closely related to the liver and spleen.The disease belongs to the mixture of deficiency and substance,with primary deficiency and evil substance as the standard.The primary deficiency is mainly qi deficiency of spleen and stomach,and the evil and abundant are mainly liver depression,qi stagnation and blood stasis.2.The common types of GIM constitution are qi deficiency,qi stagnation,Yang deficiency and blood stasis.The most common TCM syndromes are spleen and stomach qi deficiency,followed by spleen and stomach deficiency cold syndrome,liver and stomach qi stagnation syndrome,spleen deficiency dampness syndrome,liver and stomach heat stagnation syndrome,stomach and collaged-blood stasis syndrome and deficiency of stomach Yin syndrome.3,this disease should be "mutual differentiation of body syndrome,stage treatment" : in the disease attack period,should be combined with syndrome differentiation and constitution,at the same time to give constitution care;In the period of remission,physical care should be given priority,and corresponding care programs such as diet,medicinal diet,daily living,sports,music,clothing and external treatment should be provided.4.In terms of therapeutic drugs,Professor Chen often focuses on invigorating the spleen and qi,and reconcilates Yin and Yang with methods of relieving depression,promoting qi and promoting blood circulation. |
