| Objective:(1)To explore the research status and hot spots of acupuncture and moxibustion treatment of dysphagia after stroke by bibliographers,and to provide reference for the following clinical research of dysphagia;(2)To explore the clinical efficacy and safety of abdominal acupuncture combined with rehabilitation training in the treatment of dysphagia after stroke,so as to provide basis for clinical treatment of dysphagia after stroke.Methods:(1)search the related literature of acupuncture treatment of dysphagia after stroke,and extract and analyze the publication date,quantity,journals and keywords of the literature by using bibliographic co-occurrence Analysis System(BICOMB)and IBM.SPSS 25 software;(2)72 patients who met the diagnostic criteria of dysphagia after stroke was selected from January 2020 to December 2020 in the Department of Neurology of our hospital.They were randomly allocated to the observation group and the control group according to the order of admission,with 36 cases in each group.The observation group was given abdominal acupuncture+rehabilitation training+routine treatment in neurology.The control group was given rehabilitation training plus customary treatment in neurology,once a day,for 6 days a week(except Sunday),for a total of 4weeks.After treatment,the clinical efficacy of western medicine was compared between the two groups,including the NIHSS score,Wadian drinking water test,standard swallowing function evaluation scale(SSA),SWAL-QOL,TCM symptom score changes,nutritional status and adverse reactions.Results:(1)A total of 528 papers were retrieved from CNKI database,which was published in 173 journals from 1990 to 2020.154 effective keywords were extracted,involving acupuncture,combined with auxiliary,curative effect evaluation and clinical index keywords.Research hotspots of cluster analysis are displayed in five categories in turning.(2)Before treatment,baseline data such as gender,age,course of disease,number of infarctions and complications were statistically analyzed.The results showed that there was no significant differences between the two groups(P > 0.05).After treatment,the total clinical effective rates of the observation group and control group were 89% and 67%,respectively,with statistical significance(P< 0.05).Before treatment,there was no significant differences in NIHSS,Wadian drinking water test,SSA,SWAL-QOL,TCM syndrome score and nutritional status between the two groups(P > 0.05).After treatment,the grade score,SSA score and TCM syndrome score of the observation group were much lower than those of the control group(P < 0.05).The score of SWAL-QOL and nutritional status in the observation group was significantly better than those in the control group(P < 0.05).The unfavorable reaction rates of the two groups were 2.8% and 0%,respectively,with no significant difference(P > 0.05).Conclusion:(1)Bibliometric research summarizes the current research hotspots,research status and treatment methods of Chinese and Western medicine for dysphagia,but there is no literature on abdominal acupuncture for dysphagia after stroke,which can provide an opportunity for clinical trials of abdominal acupuncture for dysphagia after stroke;Summarized the clinical application of dysphagia at present,which is more representative and can provide a more comprehensive reference for the design of evaluation index scheme in follow-up clinical research.(2)Abdominal acupuncture combined with rehabilitation training can effectively improve swallowing function,nerve defect,nutritional status,TCM symptom score and quality of life of patients with dysphagia after stroke.The clinical effect of abdominal acupuncture combined with rehabilitation training on dysphagia after stroke is better than that of routine rehabilitation training,and it is economical,safe,effective and easy for patients to accept,which is worthy of further promotion in the clinic. |
