| Objective: To investigate the status of nursing safety education for new nurses, and analysis of nursing safety education needs. Then develop a curriculum system of nursing safety education for new nurses based on transition shock theory, which will provide a basis for the development of systematic and professional nursing safety training for new nurses.Methods:1.Relationship between the transition shock and patient safety competency for new nurses: From September 2020 to February 2021,a total of 339 new nurses were selected. The general situation questionnaire and The Transition Shock of Newly Graduated Nurses Scale and Patient Safety Nurse Competency evaluation Scale were used to conduct a cross-sectional survey.2.Preliminary construction of nursing safety education curriculum system for new nurses: Based on the needs of learners(new nurses)obtained from status survey and combined with the results of literature,we preliminarily established a draft curriculum system of nursing safety education for new nurses including three standard indicators and training stages. After discussion at the research team meeting,a preliminary construction of ‘nursing safety education curriculum system for new nurses’ was revised.3.Revision and completeness of the nursing safety education curriculum system for new nurses: From August 2021 to January 2022,based on the first draft of‘nursing safety education curriculum system for new nurses’,we developed the expert consultation questionnaire.16 experts were invited to conduct 2 rounds of Delphi expert consultations on the first draft through face-to-face communication or email. The nursing safety education curriculum system for new nurses was further revised and improved combined with the expert advice, and finally was established.Results:1.The score of new nurses’ transition shock was 76.38±19.802.The score of new nurses’ patient safety competency was 113.03±22.303.The variables influencing the new nurses’ transition shock included the hospital level worked, the number of hours worked per week and the times of night shifts per week. The variables influencing the new nurses’ patient safety competency included education, the hospital level worked, the number of hours worked per week, the times of night shifts per week, and the condition of training on patient safety topics. The transition shock suffered by new nurses was negatively correlated with patient safety competency(r=-0.702,P<0.01).2.Combined with the previous research results,we constructed a draft of curriculum system, which including 4 first-level indicators,19 second-level indicators, and 64 third-level indicators, and the course stage of the third-level indicators was described(divided into three stages:0-3 months,4-9 months,10-12 months after entry).3.The effective recovery rate of the two rounds of expert consultation questionnaires was 100%.The Cr was 0.90,the Kendall’s W was 0.269 and 0.250(P <0.01).After 2 rounds of consultation, the CV of each index was 0-0.21.The final version of ‘the nursing safety education curriculum system for new nurses’ included4 first-level indicators,25 second-level indicators, and 83 third-level indicators.About the course stage, the first stage of the course(0-3 months after entry)consists of 32 courses, the second stage(4-9 months after entry)consists of 42 courses, while the third stage(10-12 months after entry)consists of 9 courses. Conclusion:This study clarified the status of nursing safety education and nursing safety education needs for new nurses, and on this basis, combined with the results of literature and expert consultancy,4 first-level indicators,25 second-level indicators, and 83 third-level indicators and their course stage were determined, and a nursing safety education curriculum system for new nurses based on transition shock theory was finally formed. The curriculum system is aimed at new nurses, focus on the clinical practicality, contains the theoretical knowledge related to nursing safety and common safety risk management in clinical nursing. Also,it emphasize the cultivation of comprehensive capability of communication, cooperation, critical thinking and so on.In addition,it promote role change and responsibility cultivation, and guide new nurses to overcome role barriers. The hierarchical course stage setting is conducive to targeted and flexible education, and provided effective guidance for the systematic and step-by-step implementation of nursing safety education. Not only that, this research expanded the perspective of standardized training for new nurses, which provided reference for the formulation of standardized training program for new nurses. |
