With the improvement of people’s living standards and the liberalization of the two child policy in the family planning policy,people pay more and more attention to children’s medical health.On March 1,2019,the CPC Central Committee and the State Council proposed a new round of medical reform,in which the hierarchical diagnosis and treatment system has received great attention.In recent years,the level of medical and health services in Guangzhou has been continuously improved,but with the continuous increase of the birth population,there is still a great vacancy in children’s medical resources,and children still have the problems of difficult and expensive medical treatment.In order to solve these problems,it is of great significance to study the implementation of children’s hierarchical diagnosis and treatment policy.In order to better study the graded diagnosis and treatment of children,this paper takes district a of Guangzhou as the research object.Focus on the research on the basic situation,operation effect and main problems of children’s graded diagnosis and treatment in district a of Guangzhou,analyze the reasons behind the problems,and put forward policy suggestions to improve the graded diagnosis and treatment system for children in district a of Guangzhou.This paper mainly adopts the questionnaire survey method to investigate the understanding of children’s patients’ parents and hospital medical staff on hierarchical diagnosis and treatment,and the correlation between the first diagnosis and two-way referral at the grass-roots level.The questionnaire is analyzed by using the statistical methods of reliability,validity,correlation and linear regression to study the implementation of hierarchical diagnosis and treatment for children.Interviews were conducted at three levels: parents of child patients,medical staff and government managers.The results of relevant surveys show that although there have been some achievements since the implementation of graded diagnosis and treatment for children,there are still many problems.Using Smith’s policy implementation model,this paper analyzes and classifies the problems from four elements: idealized policy,implementation organization,target population and environmental factors.Then analyze the reasons behind the problems as important policy suggestions.Then,through the comparative method,compare the excellent experience of graded diagnosis and treatment practice in some foreign countries and some domestic cities.Finally,the research puts forward the corresponding solutions: positioning the functions of medical institutions,establishing a reasonable benefit distribution and performance appraisal system for medical institutions,actively publicizing the hierarchical diagnosis and treatment system for children,strengthening the advocacy of new ideas,and strengthening the construction of grass-roots medical service institutions and medical federations. |
