| Background: Major Depressive Disorder is a common mental disease that brings pain and burden to patients, families and society.Drug therapy combined with psychotherapy is the most recommended treatment for MDD.The application of balanced psychotherapy in the treatment of patients with MDD has proved its effectiveness. In recent years, the application of virtual reality technology in psychotherapy has gradually sprung up.This study innovatively combined virtual reality technology with balanced psychotherapy to assist drug treatment of patients with MDD,and verified its effect. In addition, this study also introduced the new concept of Allostatic load, which is a new concept of holistic medicine.Allostatic load(AL)refers to the cost of adjusting the internal environment in order to adapt to repeated and continuous external challenges. In recent years, many studies have shown that there is a certain relationship between AL and MDD,but there are few studies on the effect of AL on the treatment of MDD patients. This study confirmed the relationship between AL and MDD again, and discussed the effect of Al on the treatment of MDD.Methods: 60 healthy subjects were recruited to collect vital signs(systolic and diastolic blood pressure),blood samples and body measurements(including height, weight, waist circumference and hip circumference). The collected data were used to determine the standard of AL level;60 MDD patients who were treated in the Department of psychopsychiatry of Zhong Da Hospital Affiliated to Southeast University were randomly divided into two groups and received virtual reality based balanced psychotherapy(VR-BPT)adjuvant drug therapy or mental health education adjuvant drug therapy for a total of 8 times for 4 weeks,The collection of vital signs(systolic blood pressure, diastolic blood pressure),blood samples and body measurements(including height, weight, waist circumference and hip circumference),clinical interview and Hamilton Depression Scale(HAMD-17)were completed before the intervention. After 4 weeks of treatment,HAMD-17 was evaluated again to evaluate the curative effect. Results: There was a significant positive correlation between AL and HAMD-17 scale score at baseline in patients with MDD(r = 0.414,P < 0.05,after correction); After 4weeks of treatment, the score reduction rate of HAMD-17 in VR-BPT group was significantly higher than that in non VR-BPT group(t =-3.91,P < 0.001);AL before treatment and different treatment methods of MDD patients will affect the score reduction rate of HAMD-17 after 4 weeks of treatment(P < 0.05).The higher the adaptive load level, the lower the score reduction rate. The score reduction rate of HAMD-17 after 4 weeks of VR-BPT treatment is higher than that after 4 weeks of drug treatment alone; The level of adaptation load and the way of receiving treatment had significant effects on the classification of patients who were cured after 4 weeks of treatment(P < 0.05).Conclusion:(1)VR-BPT combined with drug therapy can significantly improve the level of depression in MDD patients compared with single drug therapy;(2)No matter what treatment method is used,AL has a negative impact on the efficacy of MDD patients. |
