| Objective: To observe the impact of surface anesthesia with aerosol inhalation of tetracaine on the dosage of general anesthesia in patients undergoing nasopharyngeal surgery and laryngeal pain after tracheal intubation. Methods:After obtaining the permission of the Ethics Committee of the Affiliated Hospital of Yan’an University,80 patients who underwent elective nasopharynx surgery under general anesthesia were selected from April 2022 to January 2023.The patients were randomly distributed into two groups:Observation Group(Group G)and Control Group(Group D),with 40 cases in each group.Group G: nebulized 5 ml of 1%tetracaine for 15 min before induction of anesthesia,Group D: nebulized 5 ml of 0.9%saline for 15 min before induction of anesthesia. The total amount of propofol,Remifentanil and rocuronium used as General anaesthetic during operation;the VAS scores of pharynx and larynx at 0h,4h,12 h and 24 h after operation;the rate of postoperative sore throat;the time duration of pain;postoperative extubation choking degree,Mean arterial pressure(MAP)and heart rate(HR)were measured at admission(T0),before intubation(T1),1 minute after intubation(T2)and 1 minute after extubation(T3).Results:(1)Age,gender,height,weight,operation duration, catheter duration and recovery duration were compared between the two groups, and there was no statistical significance(P > 0.05);(2)The heart rate and mean arterial pressure of the two groups of patients at different time points were being compared.Compared with T0,the HR and MAP of patients in group D at T2 and T3 were higher than T0,and the difference was statistically significant(P < 0.05),while the HR and MAP of patients in group G did not change significantly, and the difference was not statistically significant(P > 0.05);(3)When comparing the degree of postoperative extubation choking in the two groups, the extent of extubation choking was less in group G.The difference was statistically significant(P < 0.05);(4)When the incidence of sore throat within 24 h was compared between the two groups, the incidence in Group G was significantly lower, and the difference was statistically significant(P < 0.05);(5)When comparing the duration of postoperative sore throat, the duration in Group G was decreased, and the difference was statistically significant(P < 0.05);(6)Compared with the VAS scores of 0h,4h,12 h and24h postoperative sore throat between the two groups, the scores of group G at each time point were lower than those of group D,and the differences were statistically significant(P < 0.05);(7)The total intraoperative use of propofol, remifentanil, and rocuronium was compared between the two groups, and the difference was not statistically significant(P >0.05);(8)No adverse reactions such as drinking water choking cough, vomiting, reflux aspiration and allergy were observed in both groups.Conclusion:(1)Inhalation of 5ml 1% tetracaine before anesthesia induction cou ld not reduce the intraoperative dose of general anesthesia in patients undergoing nasopharyngeal surgery;(2)Nebulized inhaled tetracaine can be effective in suppress ing the cardiovascular response during intubation and extubation;(3)Nebulized tetra caine can decrease the incidence of extubation choking and postoperative sore thro at as well as the degree of pain. |
